• have qualifying base period wages
• be able to work
• be available to work

What do I need to file my claim?

• Social Security Number
• Driver’s License Number
• Address
• Telephone Number
• Valid email address
• Your bank routing number and account number.
• DD 214 Member 4 if you served in the military in the past 18 months
• Last 18 months of Employment
• Last Day Worked
• Reason for Separation  (e.g., quit, fired, lack of work, laid off or job eliminated, etc.)
• Separating Employer's Name, Address, Telephone Number, Fax Number (if known)

• Monday through Friday from 8:00 a.m. to 4:30 p.m. (Central time)
