Am I Eligible?

You must:

  • Lost your job due to reasons beyond your control.
  • Worked with an insured & registered employer in the Department of Labor and Human Resources.
  • Registered for job search in the Employment Services office, or any one stop centers.
  • Be ready to accept a new job when offered.
  • Had any income during at least two quarters in the last 18 months.

What do I need to file my claim?

  • Your social security number.
  • The name, address and telephone numbers of your last employer ( this information is available in your W2 form).
  • The dates you worked with that employer and the net income you earned.
  • If you worked during the week you filed your claim you must report any income earned during the week.
  • Drivers' license , identity card from another state, electoral card or any other government identification that verifies your identity.
  • Number and name of any union, if you belong to any.
  • Alien card number or visa status, if you are not a citizen of the United States, including the expiration date.
