Performs all job functions of the Behavioral Services Specialist as needed. In addition is responsible for direct clinical services to patients and families in various outpatient settings for Substance use and Mental Health Disorders.

Based upon clinical assessments and considering age-specific, developmental and cultural needs of the individual, makes recommendations to Clinical manager and Physicians to facilitate appropriate level of care.

Make also provide short-term treatment services.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

• Provides short-term psychotherapy and / or counseling for individuals, families, as needed. Supports a recovery-focused environment and patient-centered treatment approach.
• Provides patients with self-management skills and educational information so they can be full participants in their own treatment and recovery.

Assures complete, accurate, and timely documentation of clinical information in the EHR, incorporating current documentation and billing guidelines and requirements.

• Completes screenings and assessments as well as review of clients' electronic health record (EHR) and other pertinent documents.
• Helps to identify problems, offer solutions, and participate in their resolution. Maintains the confidentiality of information acquired pertaining to patient, physicians, colleagues, and visitors to Trinity Health.
• Selects and completes appropriate therapeutic interventions based upon clinical knowledge base, critical thinking, and educational background to formulate an appropriate behavioral care plan in conjunction with the CL Provider and the medical team.
• Develops goals and interventions in accordance with evidence-based practice and patient agreement.
• Assures the consistent delivery of appropriate evidence-based services to achieve measurable improvements in quality of care.
• Provides behavioral health education and utilizes community resource opportunities for the individual and / or family.
• Monitors and coordinates the delivery of health services including linking with other treatment providers and community services as appropriate
• Maintains a process for systematic follow-up by determining the effectiveness of the treatment plan with documentation of patient care outcomes.
• Reassesses and modifies the plan in collaboration with the CL Providers, medical team and with the patient as necessary to achieve treatment goals.
• Maintains strong, working relationships with other members of the treatment team.
• Provides timely feedback to the CL Providers and other treatment team members regarding patients' care, treatment recommendations, and progress through verbal communication, EHR documentation, systematic comprehensive case review, record review, and informal discussions.
• Engages in regular communication to jointly assess, prioritize, and respond to patients' care needs. Provides consultation and training to Trinity Health Ann Arbor / Livingston providers and staff.

Performs other duties as assigned.

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION / LICENSURE

Education : Master’s degree in social work

Experience : 3+ years in behavioral health / collaborative care preferred.

Certification / Licensure Must be licensed / certified in the State of Michigan and adhere to the code of ethics for area of licensure.

REQUIRED SKILLS AND ABILITIES

Training in substance use disorder is highly desirable. CAADC or Development Plan strongly encouraged. Experience in working with patients who have complex medical conditions desired.

Clinical and analytical skills necessary to assess an individual's needs for behavioralhealth services, including evaluation of the individual's risk for dangerousness to self or others.

Knowledge of Substance use disorder ASAM criteria and psychiatric diagnostic nosology and criteria.

Demonstrated competence with verbal and written communication skills that offer concise, relevant, timely communication and / or documentation of clinical assessments.

Ability to learn and utilize various software, database applications and electronic health records.

Job Summary :

Provides professional nursing services or supervises the provision of nursing services to hospice patients in accordance with accepted standards of practice and organizational guidelines.

May perform nursing services in a variety of care settings and / or roles, including but not limited to case management, admissions or on-call.

Essential Functions :

• Provides a complete physical assessment and history of current and previous illness(es).
• Completes a comprehensive initial and ongoing assessment of : Patient and family / caregiver to determine hospice needs.

The impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs, and activities of daily living, including : the risk for pathological grief, cultural and spiritual implications, and verbal and non-verbal communication patterns.

• Assists in developing, implementing, evaluating and updating the individualized plan of care; recommends revisions to the plan, as needed, based on acuity and patient / family / caregiver needs.
• Works collaboratively with the interdisciplinary team (IDT) to prioritize and provide patient and family oriented hospice services.

Communicates with the Physician, Social Worker and Spiritual Care Advisor to provide assessment information after the initial visit and with the clinical manager or designee to provide patient / family information, including unusual or potentially problematic issues, as needed.

• Provides end-of-life care including comfort care and symptom management according to NorthStar Care Community indicators.
• Provides support, follow up and education, and documents end-of-life comfort care and symptom management.
• Provides initial and ongoing patient / family instruction, education and support regarding the disease process, self-care techniques, end-of-life care, and issues of ethical concerns.

Assesses the ability of caregivers to meet patients’ immediate needs upon admission and throughout care.

• Coordinates ordering durable medical equipment and supplies.

Qualifications :

• Graduation from an accredited school of nursing with possession of a current license to practice as a registered professional nurse in the State of Michigan.
• Minimum of one (1) to two (2) years of varied nursing experience required; hospice experience and CHPN preferred.
• Ability to use technology (hardware and software) required of clinicians in their practice.
• Must be able to read, write and speak English fluently and be able to effectively communicate orally and in writing in internal and external relationships for all essential job functions.
• Must possess excellent interpersonal skills, sound judgment, effective organizational, prioritization and follow-through skills, attention to detail, tact, dependability, emotional intelligence and the ability to maintain confidentiality and promote positive, constructive relationships with communication and collaboration at all levels.
• Must be able to cope with family / caregiver emotional stress, cultural diversity and be tolerant of individual life-styles.
• Proof of current tuberculin testing required. Patient contact will not be allowed until tuberculin clearance is documented.

Connects patients with designated community agencies for resources to meet social, financial and environmental needs. The MedSW-I provides advocacy, crisis intervention, assistance to patients and families, and brokers resources in the community.
