In consultation with the Respite Care Program RN Staff, the Social Worker will screen referrals, admit and discharge clients who are homeless with acute medical conditions to the 28 bed Respite Care Program at The Salvation Army Emergency Lodge Homeless shelter;

provide case management services to eligible clients in which an individual's needs are identified and the medical, psychosocial and habilitative services designed to meet those needs are located, coordinated and monitored in collaboration with other professional staff.

About the role :

  • Collaborate with other professionals to evaluate client’s medical or physical condition and assess needs.
  • Conduct screening and intake assessments for referred clients to determine the needs of the individual and the ability of the program to meet those needs.

Includes visitations to area hospitals.

  • Collaborate with medical professionals and utilize medical social work experience to plan and coordinate client care.
  • Facilitate strong working relationships with area hospitals and other referring entities in the community.
  • Identifies medical, psychosocial, economic and physical needs of clients; assesses and reassesses client's support systems, available community resources and other factors to plan, develop, and implement an appropriate service plan.
  • Develops and implements a case plan that presents intervention strategies to meet client needs including assistance in recovery from physical illness and provide services or access to services such as benefit advocacy, financial assistance, legal aid, shelter, housing placement, employment services or education.
  • Monitor, evaluate and record client progress according to measurable goals described in case plan.
  • Refers clients to appropriate community resources; advocates on their behalf.
  • Advocate for clients to resolve their medical crisis.
  • Prepares complete and accurate case notes; writes correspondence, reports and other written materials; may prepare statistical reports and summaries;

inputs data into Service Point.

  • Reviews all assigned cases on a scheduled basis to determine continuing need for services, and present in regular case reviews.
  • Assists clients with logistics of transportation related to all medical care and appointments.
  • Accompany and transport clients to various medical appointments.
  • Assists with implementing The Salvation Army’s organizational procedures during health crises, including any necessary quarantine process and testing.
  • Attends and actively participates in meetings with other community family shelter providers on behalf of The Salvation Army Social Services.

Education : Bachelor’s in Social Work

Certifications : Wisconsin State Social Work license; valid Wisconsin Driver’s license with clearance to drive from The Salvation Army’s insurance carrier preferred.

Experience : Two years case work experience in a social service agency; medical social work experience preferred.

What will you do?

The Social Worker is responsible for assisting patients of Progressive Community Health Centers (PCHC) by creating linkages to care, providing education and self-management support.

Care linkages can include navigation to social support agencies based on identified needs or alternatively resources to mitigate the impact of social determinants of health.

Schedule : Monday - Friday, 8 a.m. - 5 p.m.

Responsibilities

  • Collaborates with PCHC providers in identifying appropriate patients in need of social work services.
  • Conducts initial and periodic assessments with patients referred to Social Worker. Prioritizes patients according to intensity, need, and required follow-up.
  • Formulates and implements a plan that addresses the patient's identified needs by assessing the patient / family needs, issues, resources, and care goals;

determining the choices available to individual patients; and educating the patient / family on the choices available.

Establishes a plan that is mutually agreed upon by the health care team and the patient / family. Evaluates the effectiveness of the plan in meeting established care goals;

revises the plan as needed to reflect changing needs, issues, and goals.

  • Identifies and effectively utilizes community resources to meet the needs of patients / families.
  • Promotes patient self-management and empowers patients / families to achieve maximum levels of wellness and independence.

Assists patients in needed items including transportation, scheduling, housing, food sources, etc.

  • Assists the patient in identifying and / or developing their support system and encourages the patient to utilize their support system.
  • Maintains accurate and timely documentation.
  • Participates in departmental and organizational committees as applicable.
  • Promotes collaborative teamwork.

What do you need to qualify?

Required Qualifications :

A master's degree in social work from an accredited program preferred. Applicants with a bachelors in social work are welcome to apply if the individual is willing to complete continuing education trainings every two years.

Required Licensing

Licensed in the state of Wisconsin as a Social Worker (APSW or obtain APSW within the first 6 months of employment if a Master level social worker).

Related Work Experience

Experience preferred with some clinical exposure, preferably in public health or community health.

Bachelors Degree in Social Work

Preferred Qualifications :

  • Prior work experience as a Social Worker in a medical setting or Federally Qualified Health Center
  • Prior work with a diverse patient population

ESSENTIAL DUTIES & RESPONSIBILITIES

Delivery of Student Services

  • Develops and implements policies, procedures, interventions and resources that maximize positive behaviors of all enrolled students and enhances school culture, safety, and achievement.
  • Maintains records on all students that have individual learning plans or have special needs or require accommodations.
  • Assesses students' needs and applies appropriate interventions and / or referrals to outside services. Assessments by school social worker generally include one or more of the following areas : adaptive functioning, behavior, social-emotional functioning and individual development.

Formal and informal assessment tools are used to conduct evaluations and screenings.

  • Provides case management services to students and / or families. This may involve collecting and maintaining a variety of student and family resources and referring students and families to community services as appropriate.
  • Provides short-term individual student counseling, initiates formal assessments to determine needs, and differentiates short-term from long-term needs and refers to outside support as necessary.
  • Manages 504 case plans.
  • Monitors student attendance and create responses for chronic absenteeism.
  • Meets with students and families on a regular basis to assess, intervene and refer to community or in-school services as needed.
  • Serves on student IEP committees and attends IEP meetings as required / requested.
  • Delivers School Social Worker services as required by student IEPs.

  • Enters case management data as required in Infinite Campus and ensures documentation of information (e.g., at risk assessment, truancy, child protective services reports, court-related documents, mental health-related documents, 504 plan information, and IEP information) is maintained in secure and confidential location.

Teacher and Staff Education and Training

  • Provides consultations to staff and teachers on social problems or mental health interventions and needs of individual students with the goal of improving classroom performance.
  • Assists teachers in identification of urgent vs. emergency service needs for students.
  • Consults with building leaders and teams regarding appropriate behavior interventions with students in the classroom setting and relevant programming based on needs assessments.
  • Consults with advisory leads to provide relevant programming based on school-wide needs assessment.
  • Collaborates and plans with Deans of Students and School Culture, RtI Coordinator and Directors of Curriculum and Instruction about specific student needs, accommodations and / or interventions.
  • Serves as state and local compliance authority on such issues as attendance, diversity, mental health, behavior management, delinquency, crisis intervention, homelessness, child abuse, neglect, and the importance of confidentiality.
  • Provides professional development for teachers and staff on behavior interventions, mental health awareness, social / emotional learning and other topics, as necessary.
  • Provides and maintains current knowledge of federal and state laws and regulations and abides by said laws and regulations with emphasis on persons with disabilities, child welfare, immigration, mental health, confidentiality, and student and parent rights.
  • Assists in creating medical alerts as necessary
  • Provides direct supervision to student social work interns, as appropriate.

Student, Parent, and Family Education

  • Promotes services to students and their families within the context of multicultural understanding and competence that enhances families' support of students' learning experiences.

  • Provides workshops for students, parents and / or families based on needs identified in Youth Risk Behavior Survey, school climate surveys, RtI universal behavior screener, and student and parent requests.
