Summary :

The LMSW Social Worker provides direct social work services to patients and their families who utilize the Emergency Department of Cook Children's Health Care System.

This position requires an assertive person who is able to communicate effectively, problem solve, and think critically. This position requires dialogue with individuals regarding difficult topics.

This Social Worker will be available to other floors during the scheduled shift to assist with any social work needs.

This is a Part-time position working two 12 hour rotating shifts per week Thursday-Sunday nights 7 : 30pm-8am

Education & Experience :

MSW / MSSW from an accredited school of Social Work.

Previous experience in a hospital setting and some prior experience working with children and families is preferred.

Licensure, Registration, and / or Certification :

Current Texas License for LMSW.

Basic Life Support Certification (BLS) for Health Care Providers within 30 days of employment.

Responsibilities

• Provides direction, supervision and evaluation of medical social services to patients in the home to ensure compliance with state practice act(s) and our policies and procedures, applicable healthcare standards, governmental laws and regulations
• Instructs, treats, observes and evaluates patients exhibiting significant social and emotional problems affecting their health status
• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems
• Participates in the development and periodic re-evaluation of the physician's Plan of Treatment for patients needing social work services
• Utilizes special skills and judgment to resolve social and emotional responses connected with the illness
• Observes, records and reports changes in patients' condition and response to treatment to Clinical Manager and the physician
• Participates in the discharge planning process
• Instructs / counsels patients and families in treating and coping with social and emotional response connected with illnesses
• Provides ongoing assessment of patient and family needs and responses to teaching
• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
• Utilizes appropriate community resources to achieve identified objectives
• Supervises, instructs and evaluates the performance of the Social Work Assistant (BSW) to assure that all medical social services are provided to patients in compliance with Company, government and professional standards
• Maintains and submits documentation as required by the Company and / or facility including any case conferences, patient / physician / community contacts, visit reports, progress notes, and confers with other health care disciplines in providing optimum patient care
• Attends, participates in and / or conducts internal staff development programs, obtains continuing education as required by Company policies and regulations
• Maintains and enhances clinical practice skills
• Maintains the highest standards of professional conduct in relation to information that is confidential in nature.
• Shares information only when recipient's right to access is clearly established and the sharing of such information is in the best interest of the patient
• Participates in Performance Improvement activities as assigned
• Communicates information that enables data collection and root cause analysis to identify opportunities for improvement
• Works under general supervision
• Abides by and demonstrates the Company Mission, Vision, and Values through both behavior and job performance on a day-to-day basis
• Adheres to and participates in the Company's mandatory HIPAA privacy program / practices and Business Ethics and Compliance programs / practices

Qualifications

• Bachelor's Degree required; Master's Degree preferred from a school of social work accredited by the Council on Social Work Education with six months' social work experience in a healthcare setting
• Current licensure / certification in the state(s) of practice if required
• CPR certification required
• Successful completion of any / all Company required pre-employment competency testing and health

clearance prior to assignment and periodically as required by Company policy and state regulations

• Good organization and communication skills and ability to function with minimal supervision also required

The Licensed Social Worker will be responsible for identifying psychosocial, mental and emotional needs along with providing, developing, and / or aiding in the access of services to meet those needs.

Services will be provided in accordance with the National Association of Social Workers (NASW) Code of Ethics, adherence to Federal, State and Local regulations and governing agencies.

QUALIFICATIONS

• Certified, licensed, or registered in the state of practice, required.
• Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling and Psychology from an accredited school of social work, required.
• One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

RESPONSIBILITIES

Completes social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs;

Completed relevant parts of the minimum data set (MDS); Utilizes the minimum data set to guide the care plan.

• Maintains accurate and timely documentation which complies with federal / state regulations, The Company Corporate policy, and specific center practices, including, but not limited to, Advanced Directives, Minimum Data Set, Social Service History and Assessment, Raps, Care Plans, Social Service Progress Notes, Behavior Tracking, monitoring of cognitive and psychosocial changes, PASARR and Discharge Planning documentation, and psych services.
• Supports each resident’s / resident’s right to self-determination; Documents resident / resident refusal of services offered;

Informs resident / resident of any therapeutic alternatives to the refused service; Involves others, as appropriate, to help educate the resident / resident about the alternatives.

• Participates in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs / issues of the resident, the goals to be accomplished for those needs / issues, and the appropriate social worker interventions.
• Ensures or provides therapeutic interventions to assist residents in coping with their transition and adjustment to a long-term care facility, including their social, emotional, and psychological needs.
• Ensures or provides support and education to residents / family members / significant others to assist in their understanding of placement and facility issues in addition to referring them to the appropriate social service agencies when the facility does not provide the needed services.
• Coordinates the resident discharge planning process and make referrals for appropriate home care services prior to the resident’s return to the community.
• Work with the interdisciplinary team and administration to promote and protect resident rights and the psychological well-being of each resident.

Prevent and address resident abuse as mandated by law and professional licensure.

• Work with residents, families, significant others and staff to provide support, information and organization for taking a more proactive role in self-advocacy to improve the quality of life / care for individual resident and those who live and work within the facility and the community.
• Assist the Social Service Director / Consultant in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility.
• Works with Director of Nursing and Administration to prepare for inspection surveys; ability to engage in interviews by inspectors, assist in immediate corrections of problems noted by surveyors, etc.

Job Summary and Qualifications

This is a PRN position

The Social Worker will assist in the facilitation of care across the continuum for selected patient care needs with a focus on the following : Evaluation of psychosocial needs, crisis intervention, development and implementation of appropriate, timely plans of care to include discharge planning for the assigned patient population.

Ability to collaborate with case managers, physicians, staff, families and community referral bases to enhance optimal patient outcomes.

Evaluation of plan of care with appropriate changes, based on plan outcomes.

What You Will Do In This Role

In partnership with RN CM, the social worker is responsible for ensuring the post-acute medical needs and level of care are appropriate

• The social worker will respond timely to referrals from RN CM or other staff when risk factors for social determinants of health are identified
• Perform thorough assessments and develop appropriate plans of care for high risk patients with discharge planning or psychosocial needs
• Perform proactive discharge planning
• Provide expertise in post-acute services and community resources
• Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, spiritual
• Provide psychosocial support and resources
• Provide education to patients and caregivers regarding community resources and post-acute level of care criteria, benefits, and coverage
• Assist in coordinating and facilitating family conferences
• Engage in community outreach and program development to support patient and caregiver needs
• Participate in interdisciplinary care planning process
• Maintain effective communication between members of the care team
• Provide expertise in ethics cases
• Assist patients and caregivers with end of life issues, from advance directives to grief support
• Provide psychosocial support to patients and caregivers for adjustment to patients' chronic and / or terminal illness / death

What Qualifications You Will Need

• Bachelor of Social Work required, Master of Social Work preferred
• Licensed Masters Social Worker (LMSW) in state of TX
• Acute care social work experience preferred
• Knowledgeable about internal, community, and post acute care resources
• Certification in social work or case management preferred
• Basic social worker skills including but not limited to empathy, sensitivity to cultural and ethnic diversity, advocacy, communication and crisis intervention
