Essential Duties and Responsibilities

  • Conducts telephone screening and interviews to determine financial eligibility.
  • Conducts an assessment of strengths and barriers to becoming self-sufficient.
  • Determines eligibility based on knowledge and compliance of Tarrant County Department of Human Services Policy and Procedures.
  • Provides individualized application packets to applicant via email, mail and office pick-up.
  • Generates Notice of Denial in the Case Management System (CMS) for the applicant. Provides explanation for denial of services.
  • Maintains appropriate documents and records with case notes.
  • Refers clients to appropriate community resources. Provides information and referral to applicants / clients to appropriate community resources such as agencies, churches, community partners and other public programs.
  • Conducts telephone screenings and interviews required to serve applicants in the call center.
  • Provides back up for the Call Center to complete telephone screening intakes.
  • Completes data entry in the CMS for telephone screening intakes and online applications.
  • Assists with agency outreach at community events.
  • Performs all other related duties as assigned.

Minimum Requirements

Requirements

Qualifications :

A Master's degree in Social Work from an accredited institution.
