The Social Worker is an integral part the care team that helps change the lives of our staff, patients and residents each day.
You will work to evaluate and ensure the care of our residents.
In this role you will :
- Support the Social Services Director with department planning and organization necessary to ensure patient / resident medically related, emotional and social needs are maintained on an individual basis
- Develop and maintain good working rapport with patients / residents, family members, interdisciplinary team members, service providers and facility staff to ensure that social services programs can be properly maintained to meet the needs of patients / residents
- Participate in the admission and discharge planning processes, conducts patient / resident assessments, care planning process and the coordination of resident room changes.
Ensures patient / resident names are on the wall plaque of their room
- Enter assessments, notes, etc. into PointClickCare and / or in department logs, Excel database, etc. Assists in maintaining department reports and records are survey ready
- Investigate residents’ complaints regarding rooms, lost items, etc. provides information to Social Services Director and assists in the resolution of complaints
- When allowance fund balance is too high, meets with residents or family members to discuss potential purchase of clothing, toiletries, etc.
that the resident may need, May also be assigned to make these purchases for residents.
Other duties as assigned
Required qualifications :
- Two years of social work experience in a long-term care facility.
- BSW or Bachelor’s degree in a related field.
- Experience using PointClickCare or other health information management system; proficiency using Microsoft Word, Excel and Outlook.
- Excellent interpersonal and listening skills, patient / resident focused, the proven ability to set priorities, problem solve and manage conflict. Benefits :

About Us
The SCHOONAMEX is operated by Specialized Education Services, Inc. (SESI). SESI Schools provide specialized education programming for K-12 students who require additional educational and positive behavioral supports to overcome challenges that impede success in a traditional school setting.
The Opportunity
The Social Worker will be responsible for providing social-emotional services for students who require additional support to thrive in school.
They will deliver high-quality services as provided by SESI and collaborate with both school personnel and families as an integrated member of the multi-disciplinary team.

Essential Duties and Responsibilities

- Provide individual, group, or push in counseling services to students in accordance with their Individualized Education Plan (IEP) or Behavior Plan
- Collaborate in the creation of IEPs, including but not limited to creating social-emotional IEP goals, participating in IEP meetings, etc.
- Complete social-emotional and behavior assessments and analyze data as needed to determine a student’s particular strengths and weaknesses, as well as develop Functional Behavior Assessments (FBA), Behavior Intervention Plan (BIP), and other transition plans as needed
- Utilize a variety of therapeutic interventions, preventative de-escalation, and crisis intervention strategies to address needs and work with a diverse group of students from varying developmental, cultural, ethnic, racial, and socio-economic backgrounds
- Support social emotional learning skill development through targeted curriculum, assessments, and more intensive behavior support as needed
- Provide in-service training to team members as appropriate, and in conjunction with any other teams including expressive therapy team and / or clinical teams
- Collaborate with external partners and providers to provide support to students, families, and staff
- Documents in students’ files according to requirements of regulatory agencies
- Light lifting, walking, and / or climbing stairs may be required
The Qualifications
- Master’s degree in social work or human services related field (mental health, psychology, counseling)
- Professional licensure including LPC or LCPC, LCSW or LMSW, and LMFT
- Minimum of two years' experience in a school, special education, or behavior health setting working with children and adolescents is preferred
