The Social Worker is an integral part the care team that helps change the lives of our staff, patients and residents each day.

You will work to evaluate and ensure the care of our residents.

In this role you will :

  • Support the Social Services Director with department planning and organization necessary to ensure patient / resident medically related, emotional and social needs are maintained on an individual basis
  • Develop and maintain good working rapport with patients / residents, family members, interdisciplinary team members, service providers and facility staff to ensure that social services programs can be properly maintained to meet the needs of patients / residents
  • Participate in the admission and discharge planning processes, conducts patient / resident assessments, care planning process and the coordination of resident room changes.

Ensures patient / resident names are on the wall plaque of their room

  • Enter assessments, notes, etc. into PointClickCare and / or in department logs, Excel database, etc. Assists in maintaining department reports and records are survey ready
  • Investigate residents’ complaints regarding rooms, lost items, etc. provides information to Social Services Director and assists in the resolution of complaints
  • When allowance fund balance is too high, meets with residents or family members to discuss potential purchase of clothing, toiletries, etc.

that the resident may need, May also be assigned to make these purchases for residents.

Other duties as assigned

Required qualifications :

  • Two years of social work experience in a long-term care facility.
  • BSW or Bachelor’s degree in a related field.
  • Experience using PointClickCare or other health information management system; proficiency using Microsoft Word, Excel and Outlook.
  • Excellent interpersonal and listening skills, patient / resident focused, the proven ability to set priorities, problem solve and manage conflict.
