As a School Social Worker at TES, you will : provide mental health and case management services regarding mental health issues, behavioral problems, areas of strength, and service placement issues & needs in order to promote improved, adaptive, & independent functioning in all domains.
form an effective treatment team with a case manager for the benefit of the client and family. provide documentation to meet all County, State, and Federal documentation requirements.
meet with the clinical supervisor on a regular basis to discuss and implement all Behavioral Board Sciences supervision requirements.
review records, organize information, and determine appropriate assessments for clients. evaluate goals and objectives on an ongoing basis and adapt them to individual needs while coordinating and designing appropriate activities, materials, and methods for remediation.
understand and comply with all TES Protocols and Procedures in relation to job expectations. travel to and attend trainings, meetings, conferences within Los Angeles County (as needed).
You will need the following minimum qualifications to be considered : A Master’s degree in Social Work and current state licensure Qualifying experience with learning disabilities, behavioral problems, and social / emotional disorders Have an understanding of state laws and professional ethics A valid driver license and hold appropriate automobile insurance with limits that allow driving a personal vehicle for work (25 / 50k liability coverage) Must clear a current background check through the FBI and DOJ A current TB Test (within the last 6 months) Preferred qualifications include : Have 1 or more of the following trainings : Child Abuse Assessment and Reporting, Human Sexuality, Alcoholism and Chemical Substance Abuse Dependency, Spousal / Partner Abuse Detection and Intervention, and Aging and Long-Term Care Experience working with individuals with special needs Environment & Physical Qualifications : Assigned work environments vary from site to site and may require services to be provided in a distance learning / therapy model.

Overview
We’re looking for a FT Hospice Social Worker (BSW) to join our team.
The Social Worker (BSW) is that member of the interdisciplinary team whose primary function is to provide psychosocial support to the patient / family unit based on an assessment that identifies the needs, goals, interventions and services indicated accordance with the established plan of care and to utilize professional training and personal judgment in monitoring the psychosocial assessment process.
About You
- Bachelor’s Degree in Social Work from a CSWE School.
- Licesure as required by the state in which the hospice is located.
- One year experience as a Social Worker in a healthcare and / or hospice setting is required; three to five years preferred
- Knowledge of terminally ill patients and their families along with Understanding of hospice and the psychosocial dynamics of illness, loss, and death;

The Social Worker is pivotal in providing counseling services and medical social work to terminally ill persons and families in collaboration with the transdisciplinary team in a variety of settings.
WHAT YOU WILL DO :
- Address the emotional, spiritual and psychosocial needs of the patient and family related to the patient’s illness, need for care, response to treatment and adjustment to care in accord with the Team’s Transdisciplinary Plan.
- Assist with group counseling and support programs for patients and families / caregivers.
- Maintain and promptly complete all required records and reports according to agency standards.
- Participate in care conferences and bereavement program.
- Assist with discharge planning.
- Assist patients and family members with referrals to community resources.
- Exhibit flexibility for emergency situations.
- Demonstrate positive customer service responsiveness according to agency standards.
- Flexible in meeting patient / family needs that require working evenings and weekends.
- Participate in the on-call rotation as scheduled.
- Assist in preparation of financial assessment and payment agreement.
- Maintain Transmission Based Precautions.
- Participate in chart review, quality improvement activities and other meetings as assigned.
- Participate in coordination of and communication with volunteer staff.
- Complete annual competency requirements.
- Assist with completion of Medicaid application process.
- Assign new cases to social workers and keep case assignment board up to date.
- Assist with orientation of new employees.
- Assist with in-service programs and outside public relations activities as requested.
- Participate on committees as appropriate or assigned.
- Supervise Social Work Interns as assigned.

SUCCESS CAPABILITIES :
- Masters prepared Social Worker preferred
- Licensed in the State of Ohio.
- Experience (2 yrs.) in counseling or related work with individuals, families and groups.
- Experience preferred in hospice, medical facilities or nursing homes.
- Knowledge of community resources.
- Must have and maintain a valid driver’s license, good driving record and auto accident insurance at a level acceptable to HWR and / or HWR’s insurer.
- Equivalent education / experience may substitute for minimum qualifications except when there are legal requirements, such as a license / certification / registration.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
