Education :

Master's Degree in Social Work or Masters of Science in Social Administration (MSSA).

Certifications :

  • Current state licensure as a Social Worker.

  • For Homecare and Hospice, eligible licensure includes LSW, LISW or LISW-S.
  • Care Management Department : Certification as a Case Manager within one year of eligibility from an approved professional organization preferred
  • Basic Life Support (BLS) through American Heart Association (AHA) required upon hire
  • For Homecare and Hospice field staff only : Valid Ohio Driver’s license upon hire; per policy 8-175 thereafter.
  • For Homecare and Hospice field staff only : Proof of automobile insurance coverage with $100,000 / $300,000 coverage, upon hire;

The Social Worker is pivotal in providing counseling services and medical social work to terminally ill persons and families in collaboration with the transdisciplinary team in a variety of settings.

WHAT YOU WILL DO :
