The University of Miami UMHC-SCCC with the Department of Social Workers has an exciting opportunity for a Full-Time Social Worker to work on the UHealth Campus.
The Social Worker provides social services to patients and their families, including counseling and support. Moreover, this position establishes a plan of care that fosters personal worth, spiritual well-being, and human dignity in each patient.
- Able to provide systematic psychosocial screening and assessments regarding mental status and psychopathology and communicating pertinent information to the healthcare team.
- On-call rotation for the facility to receive and facilitate after-hour calls.
- Able to demonstrate the ability to advocate for psychosocial-spiritual concerns and ensure that care is safe, efficient, effective, person-centered, timely, and culturally congruent.
- Documents appropriately and thoroughly in the electronic medical record.
- Formulate, develop, and implement a plan utilizing appropriate social work interventions.
- Collaborate with the health care team and involve the patient / family in the development and implementation of plans. Facilitate discharge planning across a multidisciplinary care continuum that includes interacting with consulting physicians, third party payer and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate discharge plan.
- Works collaboratively with the multidisciplinary staff to design and implement treatment plans and discharge plans for the patient.
- Be able to provide appropriate referrals depending on the needs of the patient.
- Be able to assist with financial issues.
- Facilitate relaxation exercises and assist patients and family members in achieving positive coping strategies.
- Provide education and counseling to patients and families around issues related to adaptation to the patient's illness and / or life situation.
May utilize crisis intervention, brief and long-term individual, group, and family therapies.
- Develop and facilitate support groups or special functions.
- Address and facilitate advance directives and able to assist patient / family with completion.

DESCRIPTION
The Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated.
The social worker acts as a liaison between the patient and community resources / post-acute care providers. The social worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care.
The social worker facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput.
RESPONSIBILITIES
Demonstrates responsibility and accountability in the performance of assessment, reassessment, and implementtion of the patient's discharge plan and follow up care.
Proactively communicates with the physician and healthcare team to facilitate the plan for the patient.
- Completes psychosocial assessment within one day of referral from nurse case manager. Documents the social work activity in the electronic medical record per department and organization policy.
- Maintains knowledge of pertinent legal and regulatory compliance requirements and funding for community resources. Educates patient / families andstaff on the regulatins and community resources as needed.
Seeks out opportuniites to increase knowledge of community rsources, funding and regulatory requirements.Demonstrates knowledge of human behavior.
Completes related education through computer based education or community inservice education.
- Assesses, colloborates and coordinates an appropriate discharge plan for the patient. Works with the patient / family and healthcare team to ensure that the discharge plan is ready once patient is medically cleared for discharge.
- Complies with departmental policies and procedures related to required processes including but not limited to documentation, interdisciplinary communication, IM delivery, fiscal resonsibility, and performance improvement activities as established throughout the year.
Quality, risk, and infection control issues are identified and findings sent to the personnell responsible for the same.
- Promotes the most effective and efficient use of resources to achieve the targeted length of stay. Participates in the daily unit care coordination meetings as required.
- Accepts referrals from the nurse case manager for follow up on high risk patients e.g. homeless, HIV, DCF, Baker Acts, new diagnosis and difficult transfers discharge placements.
- Works with the Social Work student program as a task supervisor or field instructor. Assumes responsibility for a special project, or assists with administrative functions.
MSW accepts field instructor responsibility for MSW level students.
QUALIFICATIONS
- Required- Bachelors in Social Work (BSW)
- Master's in Social Work(preferred), state licensure preferred (may be required by state regulations) Certification in social work or case management preferred,
- Hospital experience or health care setting, acute care social work experience highly preferred

The Family Support Specialist (Social Worker) is a grant funded position that is responsible for working in collaboration with professional social / educational services and agencies to obtain health, financial, vocational, and social services for students and families while modeling and enforcing company policies and procedures.
