The Nurse Manager (NM) is responsible and accountable for nursing operations of Mental Health Service's (MHS's) Mental Health Clinic (MHC) and Mental Health (MH) Community Based Outreach Clinics (CBOCs), ensuring that nursing personnel provide timely nursing care which complies with accepted standards of clinical practice.
The NM also directly supervises the Nursing Personnel in the MHC and CBOCs. Applicants pending the completion of educational or certification / licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements : United States Citizenship : Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Responsibilities
Job Summary
The Registered Nurse in Home and Community Based Services provides and directs provisions of nursing and
personal care to patients in their homes as prescribed by the physician or as authorized by the state / program and
in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates the plan of
care with other health care professionals involved in care and helps to achieve and maintain continuity of patient

00 - $88,000.00 Annually Shift- Day Provides, coordinates, and directs the provision of home nursing care according to physician’s orders based on agency policies and procedures, through the competent application of the nursing process.

Responsibilities Demonstrates knowledge of home care and competency in discipline specific patient care skills, required by the care center for the provision of patient care.

Demonstrates knowledge of home care and competency in discipline specific patient care skills, required by the care center for the provision of patient care.
May be assigned RN visits as member of care team, and / or may be required to supervise a patient caseload, including LPN’s and HHA’s.
Performs patient comprehensive assessments, as assigned, and collaborates with patient multidisciplinary care team (PT, OT, ST, MSW and paraprofessionals) and other health care professionals to develop / modify and implement an individualized plan of care as per physician orders that ensures quality, proper discharge planning and achieves desired outcomes and goals.
Helps to achieve and maintain continuity of patient care by communicating information (for example, changes in the plan of care, new orders, lab results, etc) timely, effectively and systematically with all team members providing care to the patient.
Facilitates interdisciplinary communication and follow-up through care coordination on assigned patients and / or caseload of patients for duration they are on service.
Provides patient episode management per company protocol. Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
Utilizes a combination of care center resources and rationally recognizes standards of practice to achieve positive clinical outcomes.
Participates in clinical outcomes monitoring, follow up and care center performance improvement initiatives. Initiates, plans and provides staff education and performs competency and skills validation for staff as needed.
Utilizes accepted company teaching materials in patient / family education. Regularly communicates patient progress to the clinical manager.
Completes documentation timely, accurately, and at the point of care, according to industry standards, conventions and guidelines, including OASIS assessments, SOC / Admissions, Recertifications, ABN’s, subsequent visits, physician orders, care coordination, etc.

Job Summary
personal care to patients in their homes as prescribed by the physician or as authorized by the state / program and
in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates the plan of
care with other health care professionals involved in care and helps to achieve and maintain continuity of patient
care by planning and exchanging information with physician, agency personnel, patient, family, case manager
and other community resources.
Specific Job Duties / Responsibilities
Provides high quality clinical services within the scope of practice and within infection control standards, in
accordance with the plan of care / service plan, and in coordination with other members of the patient / client’s
care team from admit through discharge.
Completes clinical nursing assessments in accordance with federal and / or state program requirements and as
required by payer
Ensures the patient / client’s eligibility and medical necessity for services as defined by payer source and
agency policy.
Develops and revises individualized plans of care and / or service plans according to federal and / or state
program requirements with other community providers.
Ensures plan of care frequency and duration meets the needs of the patient and initiates plan of care
revisions as needed with physician approval.
Experience Desired
A minimum of one year experience as an RN preferred
License Requirements
- Must have current RN licensure in state of practice.
- Current drivers license, vehicle insurance and access to a dependable vehicle or public transportation.
- Current CPR certification required.
State Specific Requirements
LA : At a minimum, one year of clinical experience as a Registered Nurse. One year of clinical experience as a
Registered Nurse may be waived for a Registered Nurse with recent clinical experience as an LPN. RN
licensure must have no restrictions
