3+ years' clinical experience

POSITION SUMMARY :
Under the direction of the Clinic Director, the Physical Therapist provides skilled physical therapy and rehabilitative services to patients including all aspects of evaluation and treatment assessment to patients to achieve maximum functional outcome;
improve mobility, relieve pain and disabilities. Evaluates, plans and implements specific treatment programs for individual patients according to the principles and practices of physical therapy.
Collaborates with an interdisciplinary team regarding the needs of the patient. May contribute to presentations, publications, supervision of students / interns and execution of departmental education efforts, etc.
SPECIAL REQUIREMENTS :
Knowledge of pelvic floor physical therapy techniques for both males and females. Ability to work as part of a team. Oral and written communication skills.
Problem / situational analysis. Ability to identify and seek needed information. Detail-oriented. Customer / client orientation.
Ability to recognize and report abnormal symptoms of patient conditions. Knowledge of physical therapy techniques. Adept at electronic medical records and proficient in computer skills.
RESPONSIBILITIES :
Performs evaluation and treatments of patients in accordance with established standards of care and practice including selecting assessment tools, tests and measures, goal setting, etc.
Communicates with patients, caregivers, and their families regarding the patient's progress. Develops and / or implements clinical activities which improve or expand physical therapy services to benefit patient care.
Provides supervision of physical therapist assistant(s) for provision and delivery of service that is safe, effective, and efficient in accordance to department, state, and APTA standards.
Demonstrates responsible actions and initiative to positively impact patient access to care and contribute to the Physical Therapy clinic's financial performance.
Completes accurate and timely documentation of services in accordance with professional and compliance standards to best reflect the patient's status and needs, support revenue cycle and facilitate communication for continuity of care.
Provides education and mentoring for : students, interns, residents, observers, volunteers and / or staff to support their development.
Promotes growth and development of Rehabilitation Therapy services by assisting in special projects and / or coordinating Rehabilitation Therapy activities in patient care programs.
Maintains a safe and clean working environment by complying with procedures, rules, and regulations, and adhering to infection-control policies and protocols.

POSITION QUALIFICATIONS :
MINIMUM :
A Bachelor's degree in a health-related field such as anatomy, physiology, biology, chemistry, or physics; and a graduate from a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education.
Requires Florida State Licensure as a Physical Therapist.

ABPTRFE Residency Graduate
ABPTS Board Certification in Womens Health
Other certification in Pelvic Health Training (Herman and Wallace, etc.)

The Physical Therapist :
- Under the supervision of the Empath LIFE Medical Director, provides physical therapy services to participants of Empath LIFE
- Provides clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed by Day Center, ALF, Nursing Home or in-home setting.
- Integrates the physical therapy treatment plan into the Interdisciplinary Team comprehensive plan of care.

Position Requirements
Be a graduate of a physical therapy curriculum approved by one of the following : The American Physical Therapy Association;
