As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do :
- Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
- Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
- Observe movements, conduct assessments, document progress and modify care plan as needed
- Assist patient with exercises and hands-on therapy to manage pain and increase mobility
- Educate patients on their plan of care, established goals, exercises and self-care
- Coach family on the plan of care, therapy goals and safely assisting with exercises
A few must-haves for Home Health Physical Therapists :
- Graduate of an accredited Physical Therapist Program and active PT license in PA
- Minimum of 3 years of physical therapy experience, ideally in home healthcare
- CPR certification
- Knowledge of state and federal home health regulations

Summary
Evaluates and develops plan of care and administers medically prescribed physical therapy treatments to patients suffering from injury or disease in order to relieve pain, restore maximum function, prevent disability, and promote wellness.
Minimum Qualifications
- Must possess a current Physical Therapy license, or be license eligible in the Commonwealth of Pennsylvania; licensure is required within six months of hire date.
- Basic Life Support (BLS) certification.

Provides therapy to patients. Assesses each patient, as related to the therapy requested on physician referral and provides reassessments.
Gathers data including patient past and current health issues, systems review, selection of tests and measures to elicit additional information.
Makes clinical judgments based on examination and data gathered to generate an accurate therapy diagnosis.Predicts optimal level of improvement and amount of time needed to reach that level.
Uses reexamination to determine whether predicted outcomes and goals are reasonable and modifies as necessary.Involves patient and family in establishing goals of treatment.
Designs implements and modifies therapeutic interventions.Uses re-examination to determine whether predicted outcomes are reasonable modifies as necessary.
Coordinates, communicates, documents and bills per program, regulatory and professional standards.Accommodates selection of tests and measures and therapeutic interventions to the age of the patient or client.

Job Requirements
- Bachelor's degree in Physical Therapy from an accredited program required
- 0 to 2 years in the practice or program area required
- Licensed Physical Therapist - MN State Board of Physical Therapy required if providing services in Minnesota upon hire or
- Licensed Physical Therapist - WI Dept of Safety & Professional Services required if providing services in Wisconsin upon hire and
- Valid Driver’s License required upon hire - MN and WI residents must obtain a valid driver’s license in their state of residence within 60 days of employment if providing services for Home Care and Hospice

- Maintain professional growth and development by participating in continuing education programs and other approved learning experiences.
- Maintain confidentiality of all medical record, facility, and rehab related information.
- Maintain accurate billing information in accordance with facility procedures daily.
- Attend / Lead interdisciplinary meetings as appropriate to relay clinical findings and make recommendations to other members of rehab team.
- Comply with documentation requirements of rehab department and facility.

Responsibilities
Physical Therapist responsibilities include therapy screens, evaluations, daily treatment sessions and discharge / discharge programs.
Physical demands of the position require good physical and mental health status as the position requires walking / standing most of the day, lifting, transferring, and pushing residents and equipment.
Daily documentation is done electronically via Optima Healthcare Solutions (formally Rehab Optima) with a minimum weekly productivity of 85%.
Qualifications
This position requires a master's or doctorate of Physical Therapy, State License for the state in which services are being provided and CPR.
Employee must be dependable and demonstrate the knowledge and skills necessary to provide care appropriate to the age of the residents served.
