Physical Therapist

Responsibilities

  • Assesses, plans, implements and evaluates the care provided to all patients assigned.
  • Provide client care according to plan of care
  • Reviews, interprets, transcribes and carries out physician orders
  • Prepares and maintains clinical documentation.
  • Provides legible, clear written or electronic documentation of all patient care provided, medications administered, and the patient’s response to care.
  • Implements infection control and safety precautions.
  • Attend team meetings, training, and assists teammates as needed.

PT Job Requirements :

  • Reliable Transportation
  • CPR Card (BLS)
  • TB Test (current)
  • Health Screening
  • Must pass criminal background check, abuse background check and drug screening.
  • Must provide a valid Driver’s License, current Auto Insurance, and Motor Vehicle Report
  • Current PT licensed in the state of Illinois.(must be in good standing)
  • Minimum 2 years of recent acute care and Home Health experience.

Minimum Education & Experience Requirements :

  • Masters prepared graduate with a degree in social work from a school accredited by the Council on Social Work Education.
  • State license to practice where applicable.
  • Twelve (12) months of accumulated experience within the last five (5) years.
