Primary Tasks : Medical Case Manager

  • Screen all clients for program eligibility, completes biannual reassessments for eligibility enrollment.

  • Coordinates work to achieve maximum productivity and efficiency during the assigned shift.
  • Accurately notes all patients' names by identifying them by their name and medical record number as evidenced by random observation by supervisor.
  • Properly documents required information on floor log sheet for each transport perform ancillary department as monitored by random audits of logs.
  • Check with patient's Nurse prior to the start of any activity / transport. Only transports patients with identification bracelets and following clearance from the patients' nurse
  • Correctly stores all contrast media within the department
  • Maintains patient's privacy during transport, utilizing sheets / blankets.

CORE QUALIFICATIONS :

  • High school diploma or equivalent
  • BLS Certification
  • Minimum of six months' experience as a courier in a hospital setting preferred
