JOB DESCRIPTION :

The RN CM is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team.

The RN CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team.

The RN CM will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

RESPONSIBILITIES :

  • Provides case management services for both inpatient and observation patients as assigned.
  • Identifies patients who are at risk for adverse outcomes during the transition from one level of care / setting to another.
  • Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients / family along with an assessment of resources appropriate and available to the patient / family.
  • Reassesses the patient’s clinical condition as indicated.
  • Considers patient’s readmission status or risk of readmission and develops strategies to mitigate including education on appropriately accessing healthcare resources, preventative education, and community-based resources.
  • Coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary health care team and in particular with the patient's physician to facilitate a successful care transition.
  • In partnership with Social Services, the RN CM is responsible for ensuring the post-acute medical needs and level of care are appropriate.
  • Evaluates progression of care using evidence-based tools and approved criteria (InterQual) throughout the episode of care;

escalates progression and transition of care issues through the established chain of command.

  • Makes appropriate referrals to third party payer, disease and case management programs for recurring patients and patients with chronic disease states.
  • Facilitates patient throughput with an ongoing focus on an effective care transition, quality and efficiency.
  • Documents professional recommendations, discharge plan, care coordination interventions, and case management activities to effectively communicate to all members of the health care team.
  • Adheres to established policy and procedure and standards of care; escalates issues through the established chain of command timely.
  • Demonstrates knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives Serve as an advocate for patient's rights, needs, and values;

ensuring that patients’ ethnic, cultural, or religious values, beliefs, preferences and needs are considered and aligned.

Other duties as assigned

QUALIFICATIONS :

  • RN with current state licensure; BSN preferred
  • Three years’ clinical, hospital nursing experience required with preference given to those with 2 years of case management experience or 2 years of critical care experience
  • Certification in case management or utilization review preferred
  • InterQual experience preferred

Job Summary and Qualifications

You Will :

  • Perform accurate and timely laboratory testing; analyze proficiency testing samples according to instructions and complete within the allowable time frame
  • Assume responsibility of the department as appropriate in the absence of the supervisor
  • Complete all required records / documentation pertaining to patient results and handle all problem and / or critical laboratory specimens / test results
  • Perform stat procedures within established turnaround time targets
  • Demonstrate the ability to use good judgment and independent thinking in the correlation and interpretation of patient results.

Including performing delta checks, etc.)

  • Review pending logs (OSR) and result logs; resolve any outstanding workload or communicate to staff relief
  • Perform quality control procedures per protocol
  • Take pertinent corrective action when the patient, control, standard, calibration and / or proficiency results are not within appropriate guidelines

You Should Have :

  • A 2 Year / Associate Degree is required
  • A 4 Year / Bachelor's Degree in Medical Technology is preferred
  • A State of Florida Technologist License is required OR the ability to obtain a license during the interview process
  • A minimum 3 years of experience as a Medical Technologist

DESCRIPTION :

The medical social worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated.

The social worker acts as a liaison between the patient and community resources / post-acute care providers. The social worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care.

The social worker facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput.

RESPONSIBILITIES :

  • In partnership with RN CM, the social worker is responsible for ensuring the post-acute medical needs and level of care are appropriate.
  • The social worker will respond timely to referrals from RN CM or other staff when risk factors for social determinants of health are identified.
  • Perform thorough assessments and develop appropriate plans of care for high risk patients with discharge planning or psychosocial needs.
  • Perform proactive discharge planning.
  • Provide expertise in post-acute services and community resources
  • Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, spiritual.
  • Provide psychosocial support and resources.
  • Provide education to patients and caregivers regarding community resources and post-acute level of care criteria, benefits, and coverage.
  • Assist in coordinating and facilitating family conferences.
  • Engage in community outreach and program development to support patient and caregiver needs
  • Participate in interdisciplinary care planning process.
  • Maintain effective communication between members of the care team
  • Provide expertise in ethics cases.
  • Assist patients and caregivers with end of life issues, from advance directives to grief support
  • Provide psychosocial support to patients and caregivers for adjustment to patients’ chronic and / or terminal illness / death.
  • Other duties as assigned.

QUALIFICATIONS :

  • Bachelor of Social Work (BSW) required
  • Master of Social Work (MSW) preferred
  • State licensure preferred (may be required by state regulations)
  • Certification in social work or case management preferred
  • Acute care social work experience preferred
  • Knowledgeable about internal, community and post-acute care resources
  • Basic social worker skills including but not limited to empathy, sensitivity to cultural and ethnic diversity, advocacy, communication and crisis intervention
