Summary
Under close supervision, this position provides support and structure to residents in developing program plans by managing a caseload of client cases and administering the policies and procedures of the program as directed by the contracting agency and the Facility Director.
Learns, uses and models the principles of the What Works system through evidence-based practices in all interactions with clients and other staff members.
Trains and assists less experienced case managers.
Primary Duties and Responsibilities
- May perform the duties of a Monitor including resident intakes, check-ins, breath analysis and drug screens, to provide backup when necessary.
- Prepares written personalized programs for each resident to establish measurable criteria of expected behavior and accomplishments and a time frame for achieving specified goals.
Reviews plan with residents on a weekly basis and document progress or revisions so that they are gaining the life skills necessary to make a successful re-entry into the community.
- Assists residents to establish personal budget, locate suitable housing, enroll in educational and vocational programs and participate in family and community activities, as appropriate, to ensure that they are working towards financial independence.
- Monitors facility sanitation and safety. Assigns daily housekeeping chores to residents to comply with facility standards.
- May orient residents upon arrival at facility to provide smooth assimilation into program including having them complete paperwork, take facility tours, or introduce to other residents.
- Make entries into confidential resident files to provide the structure for case analysis and program planning and to provide documentation of program participation.
Ensure files are secure to protect resident confidentiality.
Assists Job Developers to verify residents' jobs to ensure residents' employment is suitable under established program plan.
Counsels unemployed residents by providing guidance in obtaining employment in cooperation with the Job Developer.
- Verifies residents' pass plan through pass sponsorship according to contract so that the resident is accountable for his / her whereabouts in the community.
- Prepares terminal reports within established time parameters upon resident release to summarize program performance and to satisfy contract requirements.
- Participates in staff meetings by sharing observations and recommendations regarding the facility, residents and staff relations to identify and solve problems and function as a unified team.
- Maintains confidentiality obtained through job duties regarding employees, vendors, residents, and outside agencies so that sensitive information is only given on a "need to know" basis.
- Monitors client compliance with alcohol and drug counseling when required.
- Reports all program violations to the appropriate agency or official via the established reporting parameters.
- Conducts orientation meetings with new clients to explain program rules and regulations. Verbally explains the client contract in detail to ensure that the client understands his / her responsibilities.
Installs electronic monitoring equipment on new clients when necessary.
- Sets client schedules and specifies all approved activities based on sentencing information. Approves temporary schedule changes for approved activities.
- Communicate effectively with all levels of staff in a manner that promotes a team spirit and a sense of cooperation.
- Complies with work rules and policies as explained in the company Employee Handbook, and the Facility Policies and Procedures Manual;
and responds positively to directives from managers and supervisors in a cooperative spirit to ensure conformance to standards.
- Adheres to attendance policy; work overtime as required, to ensure adequate coverage of the program and to ensure the safety and security of the residents.
- Complies with safety rules; take appropriate corrective action to ensure work is performed in a safe manner and without injury to self or others.

Job Requirements
Bachelor's degree in a social, human services, criminal justice or
behavioral science program from an accredited college or
university. At a minimum, one (1) year of experience must be
working in human services, corrections, or community based
services.
Work experience may be substituted for academic
studies exchanging one (1) year of work experience in a related
field for one (1) year of academic education. Total work experience
needed in lieu of the combination of education and work
experience is five (5) years.

Case Manager
The mission of The University of Texas MD Anderson Cancer Center is to eliminate cancer in Texas, the nation, and the world through outstanding programs that integrate patient care, research and prevention, and through education for undergraduate and graduate students, trainees, professionals, employees and the public.
The primary purpose of the Case Manager position is to :
Develop an individualized plan of care by assessing, planning, facilitating and advocating for healthcare needs along the continuum of care to achieve optimal clinical, financial and operational outcomes.
Coordinates with the Multidisciplinary Care Team and internal / external customers in the management of patient care. The role integrates and coordinates utilization management concepts, care facilitations, and discharge planning functions.
Standards of Practice - Care Coordination
- Serves as the liaison between internal and external team members regarding issues related to utilization management and / or coordination of care
- Facilitates the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement

- Maintains visibility on assigned units and interacts with Clinical Care Team, patients and families
- Meets and discusses all assigned patients daily with Clinical Nurse Leader or designee
- Participates daily in multidisciplinary inpatient rounds with physician and / or designee
- Issues second IM Notice, Moon Notice, and Condition Code 44 notification and documents in OneConnect prior to discharge, as appropriate
- Responds to Patient Needs Assessment, documents interventions, and clears from list within one business day
- Responds timely to requests to coordinate discharge services for unplanned discharges
- Communicates, both verbally and in writing, all Case Management interventions regarding discharge plan

- Completes ACMA Compass training annually
Documentation
- Documents daily Case Management activities appropriately in EPIC to reflect interventions prior to the close of business day
- Documents all discharge planning activities in EPIC as needed or at a minimum weekly to include identified case management issues and progress towards discharge
- Utilizes handoff tools in EPIC
- Activates out of office notification on e-mail and voice mail with appropriate information prior to scheduled day off
- Signs in as Case Manager on all assigned patients
- Demonstrates compliance with all state and federal regulatory requirements
- Facilitates authorizations for Rehabilitation transfers and ensures the preadmission Rehabilitation MCG (Milliman) review is completed prior to transfer to the Inpatient Rehabilitation Service.
- Documents in EPIC the discharge care coordination information including name of agency with contact number, services to be provided and date and time services to start if applicable,

Case Manager
The Case Manager is responsible for directing and coordinating the functions and activities of health care services delivery and discharge planning with the multidisciplinary team.
The Case Manager facilitates communication and coordination between the health care team members involving both the resident and family in the decision-making process.

- Exhibits a positive professional demeanor to residents and relevant parties, offering constructive communication, cooperation, and assistance to ensure a satisfactory patient stay of treatment.
- Provides effective assistance and positive relations with co-workers that demonstrate collaborative multidisciplinary treatment team approach to resident or program problem-solving issues.
- Provides timely information / feedback to the designated supervisor on any resident / family / support person / payor source relevant issue that could jeopardize the patient's treatment, family / support person / payor sources, or discharge planning issue.
- Communicates with the manager and department directors to ensure team-focused patient-centered care.
