General Summary :
- Under direction of the Program Manager and with independent professional responsibility, the Case Manager II performs case management with individual program participants in permanent housing;
- performs assessments, individual counseling, group facilitation, community resource development, file monitoring and review;
reviews agency reports and develops summaries. This individual will combine a strong understanding of the housing first model and the ability to maintain a strength-based perspective to work with low-income adults from diverse social and ethnic backgrounds who have a history of homelessness.
PRINCIPAL DUTIES & RESPONSIBILITIES :
Essential Functions :
Case Management
- Perform overall assessment of program participant’s needs and develop an individualized service plans with short and long term goals.
- Review case plans weekly to adjust goals where needed or appropriate.
- Perform a wide variety of group facilitation with program participants around life skills, employment, housing, education, financial capability, nutrition and wellness, art and culture.
- Educate program participants on issues surrounding living skills, parenting, financial capability, workforce development, health and hygiene, housing, and drug and alcohol abuse.
- Develops and maintain comprehensive lists of community resources for use by program participants and refer program participants as appropriate.
- Establish individualized budgets with program participants and assist program participants in monitoring savings and expenditures.
- Document all contact with program participants; prepares all screenings, assessments, service plans, evaluations, and reporting records.
- Coordinate incoming and outgoing participant referrals with other community agencies.
- Provide crisis intervention, referrals and collaborative consultations with service providers working with the client.
- Maintain and update all client files to ensure compliance with reporting agencies.
- Transport program participants as directed following established policies and procedures.
- Attend and conduct meetings and trainings, as directed.
Other Duties / Responsibilities
- Establish and maintain positive relations with the general public, funding agency, participants and other staff.
- Ensures compliance with all Goodwill Industries Sacramento Valley Northern Nevada and Next Move policies, CARF standards, and safety and security regulations.

QUALIFICATIONS :
Basic Qualifications of the Role :
- Must have knowledge and experience with social casework objectives, principles and methods.
- Experience working with low-income adults from diverse social and ethnic backgrounds who have a history of homelessness.
- Must be able to provide program and casework consultation to other staff members.
- Must have understanding of public welfare laws, principles of community organization, and effective use of public and private community resources.
- Must have experience assessing individual and group behaviors and incorporate knowledge of social and economic forces and its effects on individual and families.
- Demonstrate ability to interview and document in the social work space.
- Must have experience and skill to establish and maintain effective individual and group relationships with participants, employees and community partners.
- Must have ability to facilitate different types of conversations with a diverse population.
- Experience using basic computer, peripheral equipment and Microsoft Office software.
- Experience in report writing and advanced file management techniques.
- No record or disclosure of criminal conviction that indicates a tendency toward theft, violence, dishonesty, deceit, drug manufacture or sale, moral turpitude, or predatory behavior.
- Must provide proof of identification and eligibility to work in the United States of America.
- Possession of a valid California Driver’s License, Class C or higher, and proof of valid automobile insurance is required at the time of appointment.
This position requires the ability to operate a motorized vehicle for the purposes of conducting agency business.

Responsibilities
Overall Responsibilities :
PCG is seeking a highly motivated and dependable Case Manager with an interest in public service who can work in our CalWORKs Employment Services Program.
These individuals will provide case management services to participants to ensure they move from government assistance to self-sufficiency.
To help our clients become self sufficient, the case managers will help job ready participants to sustain employment, and help engage our less work-ready participants in job training activities and educational programs.
This position requires full-time (Mon-Fri) in-office work. After 6 months of employment, employees may have the opportunity to work on a hybrid schedule, which will be evaluated case-by-case, based on the employee’s skills, knowledge progression at that time, and the needs of the business.
If approval is provided for hybrid work, employees may be asked at any time to return full-time to the office based on business needs.

Specific Responsibilities :
- Case management activities may include :
- Design and develop plans for participants, such as determining and assigning work activities, educational and training programs;
- Conduct assessments for barriers and skills such as education level, work history, substance abuse history, disabilities, etc.
- Determine eligibility and need for supportive services including child care, transportation, and ancillary services.
- Manage participants progress towards achieving goals by creating and updating plans.
- Assist program participants in developing vocational goals reflective of their skills, capabilities and interests.
- Monitor and record clients monthly progress to ensure compliance with program regulations and participant attendance level at assigned work activities.
- Notify countys human services specialist of non-compliance with work activities.
- Follow procedural requirement of CalWORKs by updating client case files in CalWIN.
- Maintain up-to-date knowledge of CalWORKs procedures and implement appropriately.
- Maintain up-to-date knowledge of community resources, labor market and training resources.
- Maintain in contact with each client placed in employment on a monthly basis.
- Complete all necessary county paper documents and forms.
- Maintain communication with employment organizations and educational programs where CalWORKs participants are located.
- Ensure all performance metrics are met in achieving participant stated outcomes.

- 2 to 4 years of customer relationship management or case management experience.
Compensation
Compensation for roles at Public Consulting Group varies depending on a wide array of factors including, but not limited to, the specific office location, role, skill set, and level of experience.
As required by applicable law, PCG provides the following reasonable range of compensation for this role : $22.50 - $26.
50 HR. In addition, PCG provides a range of benefits for this role.
