Overview

The Social Worker I will provide care coordination services to the patients / families of the Psychiatric Mental Health Program (PMHP).

This includes assessment of needs, development of plans, support and assistance in linking to resources and overcoming barriers to access, advocacy and psychoeducation.

Patients receiving services will be by referral from PMHP clinicians. The incumbent may engage in other social work roles within the scope of practice for a Licensed Bachelor Social Worker (LBSW) as assigned.

Responsibilities

  • Accept referrals for care coordination services according to the process designated by the care coordination manager.
  • Utilize protocols to gather information, assess needs, maintain a record of contacts, and provide a record of case activities and status through referral database.
  • Complete and submit documentation for the medical record to supervisor for approval in a timely manner.
  • Adhere to standards of care and continuing education outlined in the Case Management chapter 10.42.02 Code of Maryland Regulations.
  • Establish and maintain contact with the referring clinician, the patient, and the patient’s family while cases are open.
  • Collaborate with administrative staff to update and maintain resource and referral databases.
  • Attend and participate in staff meetings and trainings.
  • Participate on committees and workgroups within the PMHP department as assigned.
  • Participate in implementation of interventions (e.g. group sessions) with approval of the supervisor, and in collaboration with the facilitating clinician.

Qualifications

QUALIFICATIONS :

Must possess and maintain Licensed Bachelor Social Worker (LBSW) licensure issued by the Maryland State Department of Health and Mental Hygiene Board of Social Work Examiners.

EDUCATION :

Bachelor’s degree in Social Work from a program accredited by the Council on Social Work Education required.

EXPERIENCE :

  • One year of experience providing care coordination services preferred. This may include licensed, non-licensed, and / or internship experience.
  • Experience in a medical or mental health setting preferred.

The Child and Adolescent Day Hospital is a clinically intensive day program for children (ages 5-12) and adolescents (ages 13-18) with serious psychiatric problems.

Designed as an alternative to inpatient and residential care, it provides rapid assessment and crisis stabilization, and the structure and intensity of an inpatient program but for patients who do not require 24-hour treatment.

Services provided include Structured Therapeutic Group Treatment; Family Meetings conducted by a licensed clinical professional (LCSW-C or LCPC) and Psychiatric Evaluation and Medication Management provided by a staff psychiatrist.

Additional groups are provided by visiting artists from September thru May offering Drumming, Movement, Art and Drama therapies.

The Child and Adolescent Day Hospital operates 8 : 30 a.m. until 4 : 00 p.m. every Monday through Friday and is located on Sheppard Pratt's main campus in Towson (Baltimore County).

As a Social Worker / Counselor (LCSW-C / LCPC) you will :

  • Provide a wide range of social work services to an assigned caseload of patients in support of established treatment plan goals and discharge plans
  • Complete a comprehensive psychosocial assessment on each patient assigned at time of admission
  • Meet individually with patients on caseload as well as with families and / or significant others to monitor and evaluate progress
