We’re looking for a Social Worker (BSW) to join our team.
The Social Worker (BSW) is that member of the interdisciplinary team whose primary function is to provide psychosocial support to the patient / family unit based on an assessment that identifies the needs, goals, interventions and services indicated accordance with the established plan of care and to utilize professional training and personal judgment in monitoring the psychosocial assessment process.
About You
- Bachelor’s Degree in Social Work from a CSWE School.
- Licesure as required by the state in which the hospice is located.
- One year experience as a Social Worker in a healthcare and / or hospice setting is required; three to five years preferred
- Knowledge of terminally ill patients and their families along with Understanding of hospice and the psychosocial dynamics of illness, loss, and death;
- ability to apply knowledge of the special needs of hospice patient and families; sensitivity to the impact of life and death issues face by individuals with terminal illness;
tact in dealing with these patients and their families; ability to support Others at a time of crisis; ability to cope with the stress of repeated loss
This role requires full COVID-19 vaccination.

Overview
As a member of the interdisciplinary team, contributes professional social work knowledge and skills in the provision of services that support patient and family access to health care and address psychosocial factors that influence a patient’s health.
Works in various areas of the hospital providing direct care and support, transition planning and facilitation and advocacy.
Responsibilities
Patient Care
- Performs psychosocial assessment of the patient to identify priority needs, strengths, patient preferences and barriers to care.
- Provides immediate crisis intervention and support to patients / families to enhance their ability to cope with the impact of health conditions.
- Educates patient / family regarding Advanced Directives and facilitates / documents advanced care planning conversations with patients / surrogate decision makers including First Steps and IPOST / IPOLST.
- Assesses grief issues and offers bereavement support.
- Assists with planning for care transitions and collaborates with UPH, community services, and facilities to support patient safety and continuity of care.
- Completes PASRR or other screening tools when appropriate for transition to another care provider.
- Documents assessments, interventions, and referrals in the electronic health record according to documentation standards.
Education and Advocacy
- Serves as a patient / family advocate in support of patient confidentiality, informed consent, patient autonomy, and self-determination.
- Assesses patient safety to identify possible abuse, neglect or other risks to safety. Collaborates with the care team to address safety issues and files DHS reports and / or guides others in the process as mandated.
- Provides information and support with guardianship and conservatorship issues.
- Supports culturally competent services and assists with arranging interpreter services as needed.
- Provides education to the patient / family regarding available services and supports and assists the patient to access those they are eligible for.
- Provides information and education to physician and other team members in understanding the psychosocial implications of illness and disease progression for the patient / family.
- Participates in mentoring new employees and / or supervising social work interns as requested.
Care Coordination / Transition Support
- Identifies patient transitional needs by assessing psychosocial, environmental, financial and cultural strengths and barriers.
- Maintains comprehensive knowledge of community resources and acts as a liaison to refer patients / families to health and social services, health insurance, public assistance and other resources to meet patient identified needs.
- Provides expertise and plays a key role with the care team in establishing patient-centered goals of care and identifying psychosocial and behavioral strengths and barriers.
- Contributes to the comprehensive, longitudinal plan of care based on patient-centric goals and coping strategies.
- Facilitates and / or participates in interdisciplinary team meetings to review and revise the patient plan of care.
- Facilitates patient / family meetings to enhance family support of the patient’s care.
- Collaborates with social workers and other professionals across the continuum and in the community to ensure continuity of care.

Social Worker, Residential and Crisis Stabilization - External candidates are eligible for a $3,600 sign on bonus
This position provides psychosocial evaluation, therapy, discharge planning services, and support to persons residing at the Broadlawns Residential Care Facilities.
The Social Worker reports to the Residential Care Facilities Manager. They work closely and in collaboration with other residential facility staff including clerical support staff, certified med aids, residential treatment workers, service coordinators and community case managers.
Generally scheduled Monday-Friday, 8am-4 : 30pm.
Qualifications

Requirements
- Master’s Degree in Social Work, behavioral sciences or other related field
- Must be a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in the State of Iowa
- 2+ years previous experience working in care management and / or with chronic illness within a medical environment i.e.
home healt h, dialysis, hospice
- Ability to show empathy and quickly build relationships with patients and local CBOs
- Ability to occasionally visit patients or take call remotely on some nights and weekends
- Excellent verbal communication skills both in person and on the pho ne
- Ability to work with Microsoft Office and mobile phone and web-based applications
- Self-starter with the ability to work independently with minimal supervision
- Must be a road warrior! A lot of driving involved.
- Ability to travel to company main location in Nashville if necessary
