Summary

According to social work practice and professional standards, works as a member of the healthcare team to facilitate continuity of care by assessing, planning, coordinating and implementing discharge planning appropriate to the patient’s level of care and post-discharge needs.

Provides supportive counseling, crisis intervention, financial assistance, and information and referral services to both internal and external customers.

Minimum Qualifications

• Master’s of Social Work degree (MSW).
• Social Work license in the state of Pennsylvania or license eligible; must receive license within six months of hire.
• Strong verbal and written communication skills.
• Strong negotiation skills and the ability to address complex issues with patient / family and staff across multiple departments.
• Ability to work independently and to gather pertinent patient information.

Preferred Qualifications

• Minimum of 1-2 years experience in an acute care hospital setting.

Requirements :

• Licensed as Clinical Social Worker
• Active Pennsylvania License
• Comfortable With EMR Systems
• Current CPR Certification
• Computer Skills Required : Microsoft Office (Word, Excel, Powerpoint).
• Two to Four Years Related Experience or Equivalent Preferred but not Required.

Responsibilities :

• Monday Friday 8 : 30 5 : 00 P.M. EST
• Uses a variety of therapeutic techniques to provide effective services to patients suffering from behavioral health problems in areas of therapist’s competence.
• Maintain collaborative working relationship with the medical practitioners in the assessment and treatment of patients with behavioral health problems.
• Facilitates care integration amongst medical, behavioral health and other care providers.

Social Worker 2 (SW 2)- Advanced

Pay Rate : $25 / hour

Minimum Qualifications

Bachelor of Social Work from an Accredited Social Work Program or equivalent education and experience.

Basic computer skills to include, but not limited to, Microsoft Office Products.

Role Description :

Provide support services in areas such as conducting on-site monitoring reviews of long term living service providers to ensure that consumers are receiving appropriate care that meets State and Federal guidelines.

Review written documentation and meet with consumers and stakeholders to evaluate the quality of the services being provided.

Statewide travel required as necessary, including some overnights.
