CentraState Medical Center's Care Coordination department is seeking a Social Worker LSW / MSW. The Social Worker LSW / MSW provides discharge planning assistance, supportive counseling and advocacy service to assigned patients and their families.
Requires CSW or LSW along with BSW or MSW.
Responsibilities
- Assesses patient's social services and discharge planning needs and their ability to provide for those needs following hospitalization.
- Identifies situations indicating possible abuse, dependencies, growth and development problems, and sexual orientation issues and follows up appropriately.
- Provides support to patients and families coping with illness and hospitalization.
- Makes recommendations regarding needs of patients and suggestions for providing more effective comprehensive care and social work services.
- Refers patients to appropriate community agencies for concrete services following discharge.
- Provides assistance to patients and families requiring complex and comprehensive discharge planning.
- Screens patients in critical care areas weekly.
- Provides intervention for high risk patients as indicated.
- Recognizes possible cases of abuse and refers to appropriate agencies for investigation and follows-up as appropriate.
- Coordinates and administers social work student intern programs.
- Documents all assessments, referrals and interventions in the patient's medical record.
- Maintains monthly statistics with regard to patients served and completes monthly report.
- Receives formal in-service on all CentraState Nursing Care Facilities including Level of Care and admission criteria.
- Liaison to CentraState Nursing Care Facilities and monitors referral process and volume.
Qualifications
Bachelor's Social Work or Master;s of Social Work Required
Minimum of two years’ experience in medical social work strongly preferred
Certified Social Worker / New Jersey Required or Licensed Social Worker / New Jersey Required
Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team

Description
Provide varied social work (SW) services to managed care patients and their families.Aid in development of treatment plans, patient discharge needs, and provide social, emotional and mental assessments on patients.
Act as an advocate for members and their families linking them to educational resource and community resources to support their healthcare.
SW participates in GMG High Risk meeting or Case Management (CM) team meeting to optimize care plane, treatment plans and GMG protocols.
Serves as a resource to the care team.
Essential Functions :
- Serve as a strong resource to GMG physicians, clinical staff and CM staff regarding community services, resources, and facilities to best serve the patients.
- Demonstrate knowledge of utilization management, care coordination, and current standards of care as a foundation for assessment and planning activities.
- Assess the medical, physical, psycho-social and transition needs of patients and families in various settings (hospital, SNF, home, clinic, etc.
to create individualized transitional and social plans.
- Conduct periodic reassessments and adjust plans accordingly.
- Communicate effectively with patients and families to optimize the plan of care, increase customer satisfaction and minimize misunderstandings.
- Confer with physician(s), case manager, and other pertinent sources to gather and provide information to the care team.
- Communicate with the MC Medical Director as indicated on pertinent patients to discuss patient status, issues, questions.
8.Document pertinent patient / family communications, referral authorizations and plans into the EMR in a timely manner.
9.Document appropriately in complex care database and other associated work-related systems.
