FIRMAN COMMUNITY SERVICES JOB DESCRIPTION DIVISION : MATERNAL CHILD HEALTH PROGRAM : MATERNAL CHILD HEALTH SERVICES JOB TITLE : MATERNAL & CHILD HEALTH MSW / SOCIAL WORKER REPORTS TO : PROGRAM DIRECTOR Basic Responsibilities : Responsible for coordinating all psycho-social services in accordance with policies, procedures and protocol established by Maternal & Child Health (MCH) standards and licensing and other regulatory agencies requirements.

Typical Duties : Conducts a Parent Orientation and develops the Health Plan with Parents addressing the use of developmental;

screening instruments, ongoing assessment of children, and interpretation of screening results. Conducts parent education workshops based on parent’s stated need and interests.

Conducts psycho-social assessments of all participants as a part of the program’s standard intake and enrollment activities.

Responsible for the data entry of disabilities services into the Cornerstone system : including case notes, referrals for services and follow-up information.

Coordinates and monitors the disabilities services for all children enrolled in Maternal Child Health as needed. Attends workshops and shares information from workshops and trainings with parents and other staff as pertinent.

Provides annual and ongoing training. Works with MCH Nurse to assure results from ongoing developmental assessments, screenings and evaluations are shared with parents and re-screenings, referrals, and follow-ups are conducted as needed.

Acts as liaison for parents with children who need referrals for services to those services. Conducts a plan for follow-up on all referrals.

Ensures all procedures for the disabilities component are implemented, documented in the Social Service Files and entered into the Cornerstone System.

Participates in the development of program improvement plans, ongoing monitoring systems and other planning duties related to MCH.

Works with management team to develop and implement an annual and ongoing outreach and recruitment plan for identifying and enrolling pregnant moms and babies in MCH.

Monitors the Social Services Files to insure compliance. Ensures data from the Social Services files are entered into the Cornerstone System.

In consultation with parental guardians and relevant center personnel, solicits parental information, observations and develops individual or family case management plans, based upon assessed client needs.

Serves as a liaison to the clinical community, including critical support services providers. Based on indicators for Mental Health Services, convenes staffing with Director, Nurse and Case Manager and / or other Mental Health representatives as appropriate within a 30 day timeline.

Effect referral for client meeting indicators for Mental Health Services within 15 days and a timeframe not to exceed 30 days as indicated by ASQ, ESI or observation.

Conducts sessions to empower families with information on parenting, life skills, health / mental health and other services on a regular basis.

Facilitate relevant professional development workshops for all program staff. Attend IDHS, IDPH, DHS and / or other institutions’ convening meetings or staffing that are pertinent to the role, submit reports as appropriate.

Work in concert with the Program Director, MCH Nurse, and Case Management Staff. Assists in providing special help for children with atypical behavior or development.

Performs tasks with limited supervision and meet established deadlines. Performs other job-related tasks as requested by the Program Director and / or President / CEO.

Qualifications : Masters degree in Social Work, Counseling, or related field, LSW strongly preferred. At least two years experience in the field.

Must demonstrate cultural competence and responsiveness to population being served. A self-starter, flexible, and adaptable to provide service according to need of clients.

Must be able to perform in a supervisory capacity. Must be able to work independently and maintain a good working relationship with clients.

Must be able to interact successfully with adults, children and people of different backgrounds. Able to coordinate, implement and follow through.

Flexible and result-oriented. Must have driver’s license and a car with insurance. Ability to work some Saturdays. Must pass criminal background checks.
