FIRMAN COMMUNITY SERVICES JOB DESCRIPTION DIVISION : MATERNAL CHILD HEALTH PROGRAM : SUBURBAN MATERNAL CHILD HEALTH SERVICES JOB TITLE : CASE MANAGER REPORTS TO : PROGRAM DIRECTOR JOB LOCATIONS : CHICAGO / Rolling Meadows Basic Responsibilities : Case Managers provides the direct, individual, and group case management services to expectant moms with pre and post-natal care and child health services to their baby 0-1.
Additionally, Case Managers assesses the social service needs of assigned clients whose needs are of moderate complexity and handles each case within the policies, procedures and practices of the agency and works under general supervision of a Supervisor and / or Program Director.
Typical Duties : Carries a caseload of moderate complexity that is representative of the cases contracted to the agency.
Performs social work services involving the application of social work principles and theory in servicing clients within the agency or contracting agencies.
Complete a comprehensive in-home assessment or reassessment of assigned cases within the guidelines established by the agency.
Performs a variety of case management duties, such as, conducting interviews with clients and other appropriate persons for purposes of formulating treatment plans for cases of moderate complexity.
Develops a written care plan for each assigned case. Utilizes basic case management methods and techniques to achieve effective treatment goals.
Increases analytical and psycho / social diagnostic skills and resourcefulness in developing case management processes. Contact other social service agencies regarding client situations, for the purpose of making inter-agency referrals.
Provides linkage of clients to appropriate resources in accordance with specified referral procedures. Develops and maintains close working relationships with other social service agencies and community organizations in the area.
Carries out an increasing role in interpreting the services of the agency to the community and in assisting clients to utilize all available resources in the area.
Identifies and reports gaps in client services to Supervisor / Director. Reports progress on caseload to Supervisor / Director.
Performs record keeping functions as required by agency. Participate in Outreach Activities as assigned by the Supervisor and / or Program Director.
Participates in work of agency standing committees. May be requested to participate or provide assistance in other groups or other special assignments.
Attends professional development and training sessions within and outside of the agency as well as other local professional workshops to enhance job skills and understanding of social service programs.
Collaborates with program Supervisor / Director and appropriate specialist or consultants on specific case related problems.
Performs other tasks as assigned by Supervisor / Director. Qualifications : Successful completion of the requirements for a Bachelor’s degree in social work, psychology, or sociology from an accredited college or university.
At least two years experience in the field. Must demonstrate cultural competence and responsiveness to population being served.
Must have driver’s license and a car with insurance. Ability to work some Saturdays. Must pass criminal background checks.

Social Worker
FIRMAN COMMUNITY SERVICES JOB DESCRIPTION DIVISION : MATERNAL CHILD HEALTH PROGRAM : MATERNAL CHILD HEALTH SERVICES JOB TITLE : MATERNAL & CHILD HEALTH MSW / SOCIAL WORKER REPORTS TO : PROGRAM DIRECTOR Basic Responsibilities : Responsible for coordinating all psycho-social services in accordance with policies, procedures and protocol established by Maternal & Child Health (MCH) standards and licensing and other regulatory agencies requirements.
Typical Duties : Conducts a Parent Orientation and develops the Health Plan with Parents addressing the use of developmental;
screening instruments, ongoing assessment of children, and interpretation of screening results. Conducts parent education workshops based on parent’s stated need and interests.
Conducts psycho-social assessments of all participants as a part of the program’s standard intake and enrollment activities.
Responsible for the data entry of disabilities services into the Cornerstone system : including case notes, referrals for services and follow-up information.
Coordinates and monitors the disabilities services for all children enrolled in Maternal Child Health as needed. Attends workshops and shares information from workshops and trainings with parents and other staff as pertinent.
Provides annual and ongoing training. Works with MCH Nurse to assure results from ongoing developmental assessments, screenings and evaluations are shared with parents and re-screenings, referrals, and follow-ups are conducted as needed.
Acts as liaison for parents with children who need referrals for services to those services. Conducts a plan for follow-up on all referrals.
Ensures all procedures for the disabilities component are implemented, documented in the Social Service Files and entered into the Cornerstone System.
Participates in the development of program improvement plans, ongoing monitoring systems and other planning duties related to MCH.
Works with management team to develop and implement an annual and ongoing outreach and recruitment plan for identifying and enrolling pregnant moms and babies in MCH.
Monitors the Social Services Files to insure compliance. Ensures data from the Social Services files are entered into the Cornerstone System.
In consultation with parental guardians and relevant center personnel, solicits parental information, observations and develops individual or family case management plans, based upon assessed client needs.
Serves as a liaison to the clinical community, including critical support services providers. Based on indicators for Mental Health Services, convenes staffing with Director, Nurse and Case Manager and / or other Mental Health representatives as appropriate within a 30 day timeline.
Effect referral for client meeting indicators for Mental Health Services within 15 days and a timeframe not to exceed 30 days as indicated by ASQ, ESI or observation.
Conducts sessions to empower families with information on parenting, life skills, health / mental health and other services on a regular basis.
Facilitate relevant professional development workshops for all program staff. Attend IDHS, IDPH, DHS and / or other institutions’ convening meetings or staffing that are pertinent to the role, submit reports as appropriate.
Work in concert with the Program Director, MCH Nurse, and Case Management Staff. Assists in providing special help for children with atypical behavior or development.
Performs tasks with limited supervision and meet established deadlines. Performs other job-related tasks as requested by the Program Director and / or President / CEO.
Qualifications : Masters degree in Social Work, Counseling, or related field, LSW strongly preferred. At least two years experience in the field.
Must demonstrate cultural competence and responsiveness to population being served. A self-starter, flexible, and adaptable to provide service according to need of clients.
Must be able to perform in a supervisory capacity. Must be able to work independently and maintain a good working relationship with clients.
Must be able to interact successfully with adults, children and people of different backgrounds. Able to coordinate, implement and follow through.
