Responsibilities for Social Worker
- respond effectively to crises affecting students and faculty
- provide mental health and behavioral counseling to students
- recognize and appropriately take care of individual students' socio-emotional challenges
- submit referrals to outside agencies as necessary for students' well-being
- coordinate and facilitate groups for students, such as for improving social skills or addressing behavioral challenges
- complete regular reports for services provided, such as session notes and incident reports
- collaborate with administration and teachers regarding needs of students with IEPs and 504 plans
- collaborate with teaching staff, administrators, and parents / caregivers to determine how best to meet individual students' needs via needs assessments and ongoing communication
- facilitate classroom lessons regarding the holistic needs of students
- work with school faculty and staff to develop plans for maintaining the well-being of individual students and of the student population as a whole
- provide proactive services in the domains of social emotional learning, college & career readiness and academic development
- abide by the ASCA code of ethics and follow the ASCA National Model for school counseling programs
- Qualifications for Social Worker
- master's degree in counseling / Social Work
- knowledge of multiple counseling approaches
- proficiency in the use of Google Drive
- excellent verbal and written communication skills
- empathy and sensitivity to others' needs
- strong interpersonal skills
- creativity, enthusiasm, and dedication
- preferred : candidates who are practicing Catholic
- This is a part-time position for 2-3 days a week and flexible in scheduling.

We have parking lots available for staff use.

While performing the duties of this job, the employee must frequently move, stand, and lift up to 10 pounds. As well as be able to walk up and down steps several times each day and physically respond quickly in an emergency.

SUMMARY : The primary purpose of the Social Services Designee to assist in planning, organizing, implementing, evaluating, and directing the overall operation of our facility's Social Services Department in accordance with federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met / maintained on an individual's basis.
Bachelor's degree and SNF experience required
ESSENTIAL DUTIES AND RESPONSIBILITIES
Review facility policies and procedures as part of the facility's interdisciplinary team to assure compliance with state and federal regulations
Participate in reviewing and setting policies concerning resident care and quality of life
Participate in developing facility social work policies
Participate in quality assurance interdisciplinary team meetings
Understand and meet all government requirements for social service documentation
Document progress in meeting the psychosocial needs of residents
Work with the interdisciplinary team and administration to promote and protect resident rights and the psychological wellbeing of each resident.
Prevent and address resident abuse as mandated by law and professional licensure
Work with residents, families, significant others, and staff to provide support, information, and organization for taking a more proactive role in self-advocacy to improve quality of life / care for individual residents and those who live and work within the nursing home and the community at large
Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs
Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs / issues of the resident, the goals to be accomplished for those needs / issues, and the appropriate social worker interventions
Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long-term care facility, including their social, emotional, and psychological needs
Ensure or provide support and education to residents’ / family members / significant others to assist in their understanding of placement and facility issues in addition to referring them to appropriate social service agencies when the facility does not provide the needed services
Provide groups for residents’ / family members / significant others as appropriate to their needs
Provide clinical interventions to address catastrophic events that occur during the resident's stay in the facility
Coordinate the resident discharge planning process and make referrals for appropriate home care services before the resident's return to the community

A Knock At Midnight (AKAM), is looking for a part-time Counselling Social Worker to join our team and help young adults between the ages of 18 to 24 to whom we provide Information Technology training services live better lives by helping them develop problem-solving skills to address anxiety, emotional, and traumatic issues.
The Counseling Social Worker’s responsibilities include building relationships with young adults by listening to and assisting them in developing habits that improve their lives and overcome stress, anxiety, and behavioral issues.
The Counseling Social Worker will work directly with young adults to understand and address their anxiety and emotional needs to help them live healthier lives.
Counseling Social Worker’s responsibilities include :
