The community you’ll be caring for : AdventHealth Well 65+ senior care is a new kind of primary care designed with adults 65 years and older in mind.

This new and innovative way to care for the whole geriatric patient and their ecosystem provides team members with an opportunity to be a part of a game changing new part of the healthcare world.

The role you’ll contribute : AdventHealth Senior Care is looking for a Social Worker to be responsible for social work practice and performing proactive outreach to our members based on their care needs.

The primary responsibilities of the Social Worker center around problem-solving, providing resources, services, and opportunities to enhance coping capacities for members.

The Social Worker serves as the subject matter expert for the member’s psychosocial and behavioral health needs. The Social Worker also contributes to the development and improvement of social policy in the primary care setting and community.

The Social Worker is an important part of AdventHealth Senior Care and supports the care team in decision-making to provide a high-quality care experience for members.

Function as a treatment team member, interpreting social, psychological, emotional and family system problems and strengths to other members of the treatment team.

Prioritize clinical problems, formulate treatment goals and construct treatment plan, revising as needed, based on continuous evaluation and assessment of progress.

Demonstrate ability to provide social work services to member, family members and significant others in four areas : psychological functioning, adaptation to illness, injury, disability or life event, psychosocial assessment / evaluation and facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge.

Utilizes differential diagnostic skills and employs various clinical treatment interventions included but not limited to crisis intervention, solution focused therapy, individual, couples, family and group psychotherapy, grief and bereavement counseling, motivational interviewing, cognitive behavioral therapy and mental health status exams.

Identify and intervene for suspected abuse, including sensitivity to age specific concerns : physical / emotional / sexual abuse of children and adolescents : domestic abuse for adults;

caretaker / institutional abuse for senior adults.

Manage crisis situations as needed, demonstrating ability to quickly appraise crisis situation and select appropriate intervention.

Qualifications

Minimum qualifications :

Master’s Degree in Social Work and one of the following licenses :

Licensed Clinical Social Worker (LCSW)

Licensed Master Social Worker (LMSW)

Licensed Master Addiction Counselor (LMAC)

Preferred qualifications :

1 to 3 Years of Position-Related Experience Preferred

Mental Health / Addiction Certified Preferred

Broad knowledge of community resources and what is available.

Able to monitor chronic illness and med compliance.

Be able to work within community on a variety of issues and project

How you will change lives As a Social Worker at US Renal Care, you will be an integral part of a cross-functional team, working to help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation.

What you will be doing Advocate & Support. You will be part of an interdisciplinary team working to ensure patients receive the best care, including conducting all required patient assessment and care planning activities such as assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and all state / CMS regulations.

You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.

g., financial / funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies.

Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care.

You will participate in all required continuing education and staff meetings. You will collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community.

Safety & Quality. You will help with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes.

You will also participate in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with federal, state, and local laws and regulations.

The Ideal Candidate :

• At least one year experience in social work is preferred.
• Previous experience or internship in medical or psychiatric social work is preferred.
• One year post MSW experience is highly preferred and regular supervision is based on experience and licensure status per social work ladder document and educational needs.

Responsibilities :

• Provides clinical social work services to and in partnership with patients, families, and significant others. These include social work assessments, counseling, goal setting, advocacy, concrete service provision and implementation of care plans.
• Makes referrals to outside agencies and assists in the coordination of continuity of care. Works collaboratively with and provides leadership within the multidisciplinary team to optimize patient outcomes.
• Social Worker shall be highly knowledgeable about the psychosocial impact of disease on individuals and families as it relates to their age and stage of development.
• Will seek mentorship for professional growth and development.

Credentials and Qualifications :

• MSW from a CSWE approved school.
• Eligibility for licensure in the State of FL as a Licensed Clinical Social Worker is required. Employee must register with the Board of Clinical Social Work as a "provisional clinical social worker licensee" or as a "registered clinical social work intern" or submitted application to the state of Florida, if a recent graduate, within 3 months of hire.
• We will consider for LCSW, if currently licensed in the State of Florida and have two years of experience. Would prefer minimum of one year in oncology social work.

If from out of state, LCSW from that state is required and must be eligible for FL licensure.

WHAT YOU WILL DO :

The Case Manager will provide direct case management services to children, birth families, foster families and primary caregiver(s) in the home setting, day care, preschool, and / or school to ensure they receive services appropriate to their needs, and continuity of care focused on resolving or preventing the issues of child abuse or neglect.

The Case Manager will provide assessment, planning, linkage, coordination, monitoring, evaluation and advocacy to the child, family, foster family and primary caregiver.

The Case Manager will team with a therapist, foster parents, primary caregiver, guardian ad litem and other appropriate professionals.

Finally, you'll develop, strengthen and maintain a direct working relationship with mental health professionals, community agencies and the child’s own support system.
