What You Bring / Job Requirements

• MSW Required
• 6 months home care / hospice experience preferred.
• Two years post Master’s experience in health care or related field preferred

• Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart Association required for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED.

Job Summary :

Provide support and assistance to families who have diabetic children that may be facing a variety of challenges.

Key Responsibilities :

• Provides psychosocial support / intervention and care to newly diagnosed and established children with type 1 and type 2 diabetes.
• Provides the mental health assessment component in interdisciplinary care of patients and makes appropriate referrals for treatment.
• Does phone consultations with patients, other medical professionals, and protective services / Court officials to advocate around medical care of diabetes.
• A combination of previous mental health clinical experience and understanding of ethnic and economic diversity and its impact on management of chronic medical conditions is essential.
• Excellent interpersonal and communication skills are required.
• Demonstrated ability to work collaboratively is required as well as the ability to plan strategically, set priorities, and establish and achieve agreed clinic objectives.

This person must be flexible and able to adapt to changing circumstances.

• Provides ongoing education to the community, medical professionals and students.
• Contributes to new and ongoing diabetes clinical research protocols.
• Should have basic computer skills for charting / data entry of patient contacts.
• Maintains high standards of existing diabetes program
• Maintains credentials for licensure as a clinical social worker (LCSW) and, eventually as a Certified Diabetes Educator, CDE.
• Experience using evidenced based interventional in the treatment of co-morbid mental health conditions.

Job Summary and Qualifications

The Transplant Social Worker III is responsible for assessing the psychological, social, cultural and / or financial needs that impact transplant patients.

• Your role has the ability to utilize social services, counseling and clinical knowledge in the assessment of these needs.
• You are knowledgeable in ESRD / transplantation Medicare guidelines.
• You are responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patients and family needs with the efficacious and cost effective utilization of resources.
• Your role serves as a transplant social worker resource though out the facility.
• You have knowledge of payer source implications on plan of care and resources to maximize patient’s successful outcome.

What qualifications you will need :

• Licensure / Certification / Registration : Licensed Clinical Social Worker (LCSW) in the state of Colorado.
• Education : Master of Social Work (MSW) from an accredited school of social work required.
• Experience : 3-5 years recent clinical social work experience in transplantation. 5-7 years recent clinical social work in an acute setting.
• Special Qualifications : Must have excellent written, verbal and interpersonal communication skills and able to work with highly diverse population.

Must be knowledgeable of State and JCAHO standards. Must be knowledgeable of ESRD / transplantation Medicare guidelines.

Must have ability to perform multiple tasks simultaneously and work in high stress environment. Must be able to apply critical thinking and formulate diagnostic impressions based upon review of systems, history, symptoms and current presentation.

Must be able to provide crisis intervention and utilize system resources / referrals.

OVERVIEW OF THE ROLE

The primary responsibility of your job position is to assist in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service program following the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities.

The Opportunity

The Social Worker will be responsible for providing social-emotional services for students who require additional support to thrive in school.
