Responsibilities

  • Assess the psychosocial status of patients and families / caregivers related to the patient’s illness and the environment and communicate findings to the registered nurse and other members of the interdisciplinary group
  • Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse, and plan interventions based on evaluation findings
  • Counsel patients and family / caregivers as needed in relationship to stress and other identified coping difficulties, and provide crisis intervention when necessary
  • Assess for any special needs related to the culture of the patient and family
  • Maintain clinical records on all patients referred to social work
  • Participate in the development of the individualized plan of care involving the patient and family, and attend regularly scheduled interdisciplinary group meetings
  • Assist physicians and other team members in understanding significant social and emotional factors related to health problems and death / dying issues
  • Assist family and patient in planning for funeral arrangements, financial, legal and healthcare decision responsibilities
  • Ensure compliance with all state and federal legal and regulatory requirements
  • Additional responsibilities as assigned

Qualifications

  • Graduate of a Master’s program in social work accredited by the Council on Social Work Education
  • Current licensure according to Medicare guidelines to practice in state of operation
  • 1+ year experience in healthcare social work
  • Experience in home health or hospice preferred
  • Excellent verbal and written communication and organization skills
  • Commitment to clinical and documentation excellence

Under the direct supervision of the Social Service Manager and Social Service Supervisors, the Social Worker will provide SYHealth patients with support, advocacy, development, and monitoring of caseload to ensure the health, safety, and well-being of vulnerable and high-risk populations.

Essential Functions of the Job :

  • Responsible for proactive case management with the objective of improving health outcomes and providing social service support to all SYHealth patients.
  • Provided patients and families with face-to-face psychosocial assessments, interventions, support, advocacy, and case management.
  • Perform in-depth screening for internal and external community programs for the purpose of educating, navigating and connecting patients with needed services.
  • Obtain written authorization from patients and create patient profile on the Community Information Exchange (CIE) system.
  • Use active listening and conversational intent in all interactions to provide high-quality confidential connections to resources and services to all SYHealth patients.
  • Using needs assessment, identify programs and resources to assist patients based on the social determinants of health utilizing the PRAPARE Tool.
  • Actively listen to clients to develop a strategy that identifies solutions to barriers to access.
  • Develop and implement case management plan in collaboration with the patient, caregiver, physician and / or other appropriate healthcare professionals and patients support network to address their needs and goals.
  • Maintains ongoing patient case load for ongoing continuity of care.
