Responsibilities
1. Independently provides social work services in the form of casework with individuals, couples, or families. Acts as therapist in support groups for patients.
a. Coordinates and manages continued care in dispersed locations, including remote regions of the United States and in foreign countries, where health care resources may be scarce or unavailable;
locates and arranges for care with civilian providers, creatively and independently designing methods of payment and service delivery.
Duties include formulating agreements with non-TriCare vendors, foreign nation organizations, foreign governmental entities, other non-local Military Healthcare System resources, the Veteran’s Administration health care system resources, and charitable or other private funds.
Incumbent provides direct social work practice in cases with complex problems, organizes community services on behalf of beneficiaries, develops and coordinates procedures for the use of community services by related staffs and satellite facilities, and develops and maintains working relationships and agreements with other organizations having responsibilities for the same groups of people.
b. Conducts psychosocial evaluations of inpatients and outpatients, assisting in the development of multidisciplinary treatment plans with specific emphasis on complex cases that focus on high risk individuals that require a high level of professional skill and judgment.
Such complex cases include, terminal patients, patients with life threatening and disabling illnesses, physically abused or neglected adults, patients who live alone and are unable to care for themselves and patients who lack capacity to make their own treatment decisions.
c. Incumbent provides brief strategic and medical crisis counseling for patients and families in coordination with the primary medical team and adjunct consulting services.
Crisis counseling involves therapeutic intervention with individuals, families and / or groups to lessen the impact of acute, chronic and / or terminal illness or injury on the lives of patients and families.
d. Provides individual psychotherapy and family therapy as indicated under the direction of the Service Chief, Social Work Department, or his / her designee.
Also, provides evidenced-based treatment modalities that are specific and appropriate to each patient’s circumstance.
e. Coordinates and implements discharge plans by arranging for transition to the next level of medical care in compliance with the multidisciplinary treatment plan.
Maintains resource information for referral to civilian agencies as appropriate.
f. Functions as a member of the various medical or surgical teams, presenting reports at team conferences and participating in evaluation and treatment planning for patients.
g. Plans, coordinates, and provides comprehensive referral services to patients and families. The employee is allowed freedom of action and exercises mature professional judgment within the treatment process, dealing constructively and definitively with the patient’s assessed issues.
h. Incumbent is responsible for serving various beneficiary groups scattered over a large geographical area. Assignments include direct social work practice in cases with complex problems, organization of community services on behalf of beneficiaries and development and maintenance of working relationships and agreements with other organizations having responsibilities for the same groups of people.
i. When assigned, carries on-call pager to be reached any time after regular working hours to assist the treatment team with emergent patient dispositions, patient related psychosocial concerns, and other complex psychosocial issues related to the medical condition.
2. Incumbent is responsible for providing orientation to stakeholders and recipients of social work services. Incumbent provides in-service training about complex social work services to multi-disciplinary treatment teams.
Incumbent educates medical staff on the psychosocial impact of injury, discharge procedures and community treatment resources.
Also, incumbent provides training for paraprofessionals including : social work students, social work interns, social work fellows, and mental health specialists.
As assigned, serves as a clinical supervisor for MSW students, post-MSW Interns and / or post-licensure Fellows.
Performs other duties as assigned.

Principle Accountabilities :
- Coordinate appropriate referrals to community and other Government agencies for services.
