While the face of the base is familiar to many who have watched it grow for years, the infrastructure has evolved with, and anticipated, developments in technology, service, and family needs.

In 1959 the Main Gate to the base was moved to accommodate a growing base and improve security; in 2000 it was moved again for the same reasons.

  • The golf course on base was opened in 1960 and underwent rehabilitation in 2000. In 1956, USS Franklin D. Roosevelt (CVA 42) was home ported in Mayport;
  • in 2000 USS Roosevelt (DDG 80) brought the Roosevelt family name back to Mayport. In 1965, the Carrier Pier Snack Bar was opened;

in March 2000, a new Single Sailor Center opened for the Mayport population of single Sailors. Named Planet Mayport, it provides a computer / net surfing room, computer game area, large screen TV room, movie theater, laundromat, ATM, library, and many other amenities.

The base continues to rehabilitate and modernize its housing areas; its piers underwent renovation; and the Morale, Welfare, and Recreation team continues to provide the base population with the best in entertainment, value, and convenience.

Responsibilities

As a Physician Assistant (Family Practice) you will perform duties such as :

  • Provide consultation, teach, and coordinate with patients and their family members of treatment and medical care.
  • Examine patients and perform initial patient work-up and assessment of patients' care and treatment.
  • Determine the need for and orders laboratory tests, X-rays and other procedures necessary to complete assessment of the patient.
  • Prescribe for and treat a variety of diseases, disorders, and injuries.
  • Provide a full range of emergency services or crises interventions in the absence of the physician.

Qualifications

US Citizenship Required

At least one year of experience as a certified physician assistant treating patients in a Family Practice setting.

Graduation from a physician assistant educational program, accredited by the Accreditation Review Committee on Education for the Physician Assistant (ARCPA), or its predecessors, at a college, university, or educational institution that is accredited by an accrediting body or organization recognized by the Secretary, U.

S. Department of Education.

Successful completion of the National Commission of Physician Assistants (NCCPA) examination; must maintain status as a certified physician assistant, including completion of all requirements for continuing medical education (CME) and re-certification.

Responsibilities

Receives general supervision from the assigned program director or Department Chief who allows the incumbent freedom of action as an independent professional in dealing with assigned caseload.

Incumbent is expected to make decisions and recommendations with a minimum of supervisory guidance. When consultation is required on difficult cases the immediate supervisor will be consulted or a designated supervisory staff to assist.

Work will be reviewed by peer reviews managed by the department in which the program falls within.

INTRODUCTION

Serves as a social worker providing individual, family, and group clinical counseling services using various evidenced based modalities as clinically indicated for beneficiaries, adults, which may include children.

Assists clients to identify and manage personal, psychological and interactive problems, learning disabilities, and behavioral problems associated with occupational, school, family, and personal adjustment.

Develops individualized treatment plans based on psychiatric diagnosis, identified problems and treatment needs for each case.

Utilizes social work systems and psychological theories, principals and techniques in treatment plan development. Coordinates with patients work environment, social service agencies, schools, primary care, specialty clinics and other organizations to promote seamless service and support to patients.

Provides limited case management to improve individual family and occupational functioning and mitigate crises. Works closely with multi-disciplinary treatment staff to synchronize efforts across the continuum of care.

Provides emergency on-call crisis management services for adult and child behavioral health cases as designated by the section or department needs.

Responsible for documentation which includes : monthly reports on patients seen outcome / assessment results, clinical issues of significance, and other data as requested.

MAJOR DUTIES

Works with highly complex cases in which individuals are severely impacted by moderate to severe psychiatric diagnoses to include factors such as history of trauma, co-occurring conditions and suicide.

Provides individual, family, and group clinical counseling services using various evidenced based modalities as clinically indicated for adults and children to assist clients to identify and manage personal, psychological and interactive problems, and to achieve effective personal, occupational, marital, and family development and adjustment : Collects bio-psycho-social information about patients using interview, case history, and observation techniques, and assessment methods.

Analyzes information collected to determine psychiatric diagnosis, treatment plan and / or advisability of counseling or referral to other specialists or institutions.

Reviews notes and information collected to identify problems and concerns. Consults reference material, for example the most current version of the Diagnostic and Statistical Manual for Mental Disorders (DSM), textbooks, manuals, and journals, to identify symptoms, make diagnoses, and develop therapeutic or treatment plan.

Counsels’ clients, using counseling methods and procedures using approved evidence based treatment modalities to assist clients in gaining insight into personal and interactive problems, to define goals, and to plan action reflecting interests, abilities, and needs.

Evaluates results of counseling methods to determine the effectiveness of treatment used. Interacts with other professionals to discuss therapy or treatment, new resources or techniques.

Provides case management services to effectively manage patients enrolled in behavioral intensive outpatient behavioral programs and prevent crises.

Regularly coordinates with other clinics, family advocacy program, Army Substance Abuse Program, installation chaplain services, Service Member commands, social service agencies, schools, and other organizations that support the patient.

Where such collaboration does not already exist, takes the lead in developing inter-agency and / or interdisciplinary forums.

Continuously assesses and responds to changing needs of patients based on deployment cycles or critical events.

Executes performance initiatives through revision of clinical standards and procedures. Performs a variety of quality management / quality assurance duties to meet the Joint Commission (TJC) requirements.

Establishes and monitors objective behavioral outcome measures. Monitors risk management issues and takes appropriate actions.

Provides concise documentation of care and recommendations. Provides reports and approves consults. Skill in written and oral communication especially in case formulation and presentation.

Performs other duties as assigned.

This is an inclement weather position. The incumbent is expected to make every attempt to report for duty on time and / or remain on duty during inclement weather conditions.

Incumbent may be required to perform call duties; work overtime to include weekends and federal holidays, out of cycle, or unscheduled training or on an emergency basis.

The work will require the incumbent to work evenings / nights, rotating shifts, extended hours or other schedules common to hospital operations.

This position requires occasional TDY - 5%.

This position has a mandatory seasonal influenza vaccination requirement and is therefore subject to annual seasonal influenza vaccinations.

Responsibilities

1. Independently provides social work services in the form of casework with individuals, couples, or families. Acts as therapist in support groups for patients.

a. Coordinates and manages continued care in dispersed locations, including remote regions of the United States and in foreign countries, where health care resources may be scarce or unavailable;

locates and arranges for care with civilian providers, creatively and independently designing methods of payment and service delivery.

Duties include formulating agreements with non-TriCare vendors, foreign nation organizations, foreign governmental entities, other non-local Military Healthcare System resources, the Veteran’s Administration health care system resources, and charitable or other private funds.

Incumbent provides direct social work practice in cases with complex problems, organizes community services on behalf of beneficiaries, develops and coordinates procedures for the use of community services by related staffs and satellite facilities, and develops and maintains working relationships and agreements with other organizations having responsibilities for the same groups of people.

b. Conducts psychosocial evaluations of inpatients and outpatients, assisting in the development of multidisciplinary treatment plans with specific emphasis on complex cases that focus on high risk individuals that require a high level of professional skill and judgment.

Such complex cases include, terminal patients, patients with life threatening and disabling illnesses, physically abused or neglected adults, patients who live alone and are unable to care for themselves and patients who lack capacity to make their own treatment decisions.

c. Incumbent provides brief strategic and medical crisis counseling for patients and families in coordination with the primary medical team and adjunct consulting services.

Crisis counseling involves therapeutic intervention with individuals, families and / or groups to lessen the impact of acute, chronic and / or terminal illness or injury on the lives of patients and families.

d. Provides individual psychotherapy and family therapy as indicated under the direction of the Service Chief, Social Work Department, or his / her designee.

Also, provides evidenced-based treatment modalities that are specific and appropriate to each patient’s circumstance.

e. Coordinates and implements discharge plans by arranging for transition to the next level of medical care in compliance with the multidisciplinary treatment plan.

Maintains resource information for referral to civilian agencies as appropriate.

f. Functions as a member of the various medical or surgical teams, presenting reports at team conferences and participating in evaluation and treatment planning for patients.

g. Plans, coordinates, and provides comprehensive referral services to patients and families. The employee is allowed freedom of action and exercises mature professional judgment within the treatment process, dealing constructively and definitively with the patient’s assessed issues.

h. Incumbent is responsible for serving various beneficiary groups scattered over a large geographical area. i. When assigned, carries on-call pager to be reached any time after regular working hours to assist the treatment team with emergent patient dispositions, patient related psychosocial concerns, and other complex psychosocial issues related to the medical condition.

2. Incumbent is responsible for providing orientation to stakeholders and recipients of social work services. Incumbent provides in-service training about complex social work services to multi-disciplinary treatment teams.

Incumbent educates medical staff on the psychosocial impact of injury, discharge procedures and community treatment resources.

Also, incumbent provides training for paraprofessionals including : social work students, social work interns, social work fellows, and mental health specialists.

As assigned, serves as a clinical supervisor for MSW students, post-MSW Interns and / or post-licensure Fellows.
