Additional responsibilities include :
- planning and implementing a comprehensive social services program that offers counseling and other support services for residents and their families
- evaluating and being keenly aware of residents’ medical-related social needs and then formulating the appropriate plan for treatment that fits into their total care plan
- coordinating and implementing family meetings for residents and creating a forum for ongoing discussions between residents, their families / responsible parties and the treatment team
- maintaining residents’ rights at all times while liaising between all parties involved

JOB REQUIREMENTS :
- Bachelor’s degree or higher in social work or human services (knowledge of family assessment and interventions is preferred)

- Excellent command of the English language (written, read and spoken), excellent interpersonal skills and knowledge of all governmental regulations relating to the social services department
- Ability to work independently, problem solve and makes decisions as necessary

JOB SUMMARY :
Develops and maintains an environment which supports service excellence behaviors.
Provides a safe environment for patients, visitors and / or employees by being aware of the safety and health policies of the hospital and adhering to and enhancing these policies via the identification / reporting / correcting of unsafe conditions.

The Social Worker will assess the patient’s psychosocial problems and needs within 72 hours of admission and ongoing thereafter.
In collaboration with the patient, family / significant other and other members of the treatment team, the Social Worker will develop an initial and ongoing plan for treatment and aftercare that is based upon the identified, prioritized psychosocial problems.
The Social Worker carries out the plan for treatment and aftercare as designed, evaluates the patient’s responses to the planned interventions, and makes decisions regarding continuation or revision of the treatment plan in coordination with the treatment team
The Social Worker maintains the ability to interact with clients, families and co-workers for the purpose of fostering a therapeutic relationship with patients.
The Social Worker documents all phases of the treatment process in the medical record within appropriate time frames.
The Social Worker is clinically and administratively responsible to the Director of Mental Health Services
The Social Worker maintains accountability for clinical competency, professional growth and development and the professional ethics of clinical practice.
The Social Worker demonstrates the competencies necessary to provide services to the psychiatric patient.
The Social Worker is responsible for maintaining the appropriate documentation for the position.

Social Worker I
Responsibilities
The primary purpose of the Social Worker I position is to be skilled in interventions for the support and / or resolution of patient and family crises as well as advocacy and facilitation necessitated by life-changing events.
The role includes other functions such as discharge planning. The position functions with considerable autonomy in providing professional social work treatment and services to patients from newborn to elderly and understands the life cycle changes consistent with each age and their families related to their health care and general well-being.
Qualifications
Possession of a Master’s Degree in Social Work (MSW) from a professional school accredited by the Council of Social Work Education and two (2) years of post MSW experience in a medical setting preferred.
Must be Licensed as an LSW Licensed Social Worker or LCSW Licensed Clinical Social Worker licensure by the State of New Jersey.
