Social Worker

Alaris Health at Kearny, a 120 bed-skilled nursing Center in Kearny, New Jersey, seeks a social worker to plan and implement social service programs in the Center and coordinate discharge planning.

Additional responsibilities include :

  • planning and implementing a comprehensive social services program that offers counseling and other support services for residents and their families
  • evaluating and being keenly aware of residents’ medical-related social needs and then formulating the appropriate plan for treatment that fits into their total care plan
  • coordinating and implementing family meetings for residents and creating a forum for ongoing discussions between residents, their families / responsible parties and the treatment team
  • maintaining residents’ rights at all times while liaising between all parties involved
  • coordinating discharge planning and assisting in the creation of an organized discharge plan for all residents
  • and above all, ensuring that residents and their families receive the highest quality of service in a caring and compassionate atmosphere that recognizes the individuals’ needs and rights

JOB REQUIREMENTS :

  • Bachelor’s degree or higher in social work or human services (knowledge of family assessment and interventions is preferred)
  • Certifications required by New Jersey state regulations
  • Excellent command of the English language (written, read and spoken), excellent interpersonal skills and knowledge of all governmental regulations relating to the social services department
  • Ability to work independently, problem solve and makes decisions as necessary
  • Ability to positively interact with everyone from residents to government agencies and the general public

Additional responsibilities include :

  • assisting in planning and promoting recreation based on individualized needs and interests and encouraging participation and engagement
  • visiting bed-bound residents and non-participants and providing appropriate recreation services for them
  • assisting with the daily radio program, monthly newsletter and recreation calendar
  • assisting with volunteer program / appreciation set-up, admission assessments, clinical responsibilities and community interaction programs
  • and above all, ensuring that residents and families receive the highest quality of service in a caring and compassionate atmosphere that recognizes each individual’s needs and rights

JOB REQUIREMENTS :

  • Bachelor’s degree or equivalent and certification in accordance with regulatory agencies governing the Center

Additional responsibilities include : .

  • Makes rounds on unit and assures that all resident rooms are clean and orderly
  • Assist nursing with tray pass and collection
  • Makes both occupied and unoccupied beds and coordinates with all departments to ensure a timely sanitation of rooms upon admission and discharge
  • Provides snack / water at bedside in accordance to the resident’s individual dietary restrictions and needs
  • Assist in transporting of resident to rehab therapy, activities, and to various areas in the facility

HOSPITALITY AIDE JOB REQUIREMENTS :

A minimum of a high school education or high school equivalency diploma is preferred for this position
