General Summary :

Under the supervision and direction of the Medical Director, Department of Emergency Medicine, the Senior Administrative Director, and the Chief Physician Assistant, the Physician Assistant (PA) is responsible for the care of emergency department patients under the direction of the attending physicians and in accordance with the rules and regulations of Massachusetts governing Physician Assistant practice.

The PA will obtain patient histories, perform comprehensive physical examinations to evaluate patients for admission, perform bedside diagnostic and therapeutic procedures, coordinate tests and consultations, implement treatment plans, communicate with referring and consulting physicians, perform patient education, and prepare patients for discharge.

The PA assumes responsibility for patients in accordance with service policies and protocols. New Graduates are not being considered at this time.

Principle Duties & Responsibilities

Can provide care in all areas of the emergency department, in conjunction with emergency medicine staff. Includes on-going evaluation and care of patients, rounds with staff, and safe and effective sign-out of patients at shift change.

  • Works closely with emergency department patients and their families.
  • Works as a member of the emergency team including attending physicians, residents, nurses, students, and ancillary personnel.
  • Obtains a detailed, accurate medical history and performs a comprehensive physical examination on patients.
  • Performs examination and evaluation of emergency patients and reinforces information presented by the attending physician to the patient and / or family as necessary.

Orders necessary diagnostic tests, consultations, and procedures as indicated or requested by the attending physician. Prescribes medications as necessary.

  • Performs or assists in routine and complex procedures as necessary, including but not limited to splinting, arthrocentesis, relocation of joint dislocations, laceration repairs, abscess incision and drainage, orogastric and nasogastric intubation, nail trephination, insertion of urethral catheters, collection of arterial blood samples, removal of nasal, ear and wound foreign bodies, anterior nasal packing, slit lamp examination, local nerve anesthesia and nerve blocks.
  • Writes complete and comprehensive notes on all patients using the EPIC system in a timely manner and discusses the care of each patient with the attending physician and other consult services that are involved with the care of that patient.

Maintains communications with referring physicians.

  • Performs basic and advanced cardiac life support (ACLS) procedures as necessary and directs these efforts until the arrival of an attending physician.
  • Prepares emergency patients for discharge, including patient and family discharge teaching, continuing care and rehabilitation arrangements in conjunction with nursing, writing discharge prescriptions, and confirming primary care or referring physician follow-up.
  • Prescribes medications for emergency patients in accordance with service polices and / or attending physician's instructions.

Uses e-prescribing function in EPIC.

  • Under the direction of the attending physicians, executes a wide range of management plans for emergency patients.
  • Writes formal discharge summaries of emergency patients in EPIC.
  • Participates in the Department of Emergency Medicine educational meetings with students and residents assigned to the service.
  • Responsible for own professional growth and maintenance of required credentials. Maintains the standards and educational requirements specific to the position and licensure.

Identifies own learning needs and goals. Maintains and updates clinical knowledge and skills based on current health care practice / research.

Skills & Competencies :

  • Qualified in all basic and advanced cardiac life support (ACLS) standards as established by the American Heart Association.
  • Possess the ability to care for critically ill patients with a broad range of medical problems.
  • Possess the maturity, self-confidence, and ability to follow complex patient management plans as directed by the attending physician.
  • Eligible for state and federally controlled substances registration.
  • Diagnose and treat health problems; obtain patient histories, perform comprehensive physical exams, do simple diagnostic tests, prepare a diagnosis and implement treatment procedures for illnesses;

and prescribe medications.

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his / her assigned unit.

Qualifications

Licenses :

  • Required : Graduate of an accredited American Medical Association-approved Physician Assistant training program.
  • Required : Certification by the National Commission on Certification of Physician Assistants within one year of hire. Must maintain this certification by completing required 100 hours of Continuing Medical Education credits every two years.

In addition, PA must demonstrate continued competence by passing a re-certification exam every six years.

Job Title : Home Hospital Advanced Practice Provider

General Overview

The Home Hospital Advanced Practice Provider (HH APP) will be part of a clinical team that provides acute-level care to patients in the comfort of their own home.

The MGB Home Hospital (HH) is an innovative strategic program that aims to improve the care of patients while increasing hospital capacity and reducing total costs of care.

The Home Hospital Advanced Practice Provider will support the work of Home Hospital by visiting (either in person or virtually) and managing patients in their home as a direct Home Hospital provider, supported by a multidisciplinary team that includes an attending physician (who oversees the direction of care for patients enrolled), a Home Hospital nurse, a paramedic (Mobile Integrated Health), an administrative coordinator, and other allied staff as needed (, physical therapist).

Upon admission to Home Hospital a patient will have an in-person visit (this can be done in the hospital or in the home).

Daily Home Hospital rounds for the Home Hospital APP consist of at least one daily visit (in-person or virtual depending on patient needs and circumstances), with medical decision-making and delivery of needed care as appropriate (see specific clinical duties below).

The HH APP will facilitate continuity between care settings, ensure home care and acute care protocols are seamlessly integrated in the home settings, and function as a lead clinician in the home environment.

The ideal candidate for this HH APP role will be a patient-centered, proactive, flexible, and confident clinician comfortable with autonomy, multidisciplinary team collaborations, complex clinical decision-making, level-of-care triaging, and telemedicine.

The role offers the potential to grow into a managerial / leadership position and / or the opportunity to broaden clinical duties.

Principle Roles / Responsibilities :

Provides acute-level care in the home setting (in-person and / or virtual) as a key member of the Home Hospital treatment team (including receiving the patient at his / her / their home, establishing a treatment plan, following Home Hospital protocols where appropriate, liaising between the home and other care settings, timely clinical documentation, and responding to patient needs with the ability to triage decisions around escalation of care).

Other clinical duties may include facilitating in-home diagnostics, point-of-care testing, administration of medications and other treatments, patient monitoring, clinical documentation, and other patient needs as they arise.

Collaborates with multidisciplinary Home Hospital team members and patients’ longitudinal care team members around appropriate, safe, effective, equitable, and timely care delivery for Home Hospital patients.

Communicates with the patient and family about HH, incorporating patient and family values and goals in the plan of care.

Maintains a safe care environment for HH patients and leads in the practice of and reinforcement of a strong safety culture among all clinical partners involved.

Attends relevant team and other meetings related to Home Hospital and related work.

Participates with quality measurement and / or quality improvement initiatives, as appropriate, to advance the work of Home Hospital and other alternative care pathways, including advancements or innovations within the electronic health record.

At least 3 years of Nurse Practitioner (NP) or Physician Assistant (PA) experience strongly desired, preferably practicing in an acute hospital setting;

academic medical center experience a plus.

Current Massachusetts Licensure as a NP or PA with a Masters Degree required.

MGB institutional knowledge a plus.

Advanced Cardiac Life Support (ACLS) certification is required.
