Job Summary

The Physician Assistant provides clinical care to patients through an interdisciplinary and collaborative approach that takes into consideration all aspects of health.

A Physician Assistant performs a variety of advanced clinical duties involved in assessing, treating, controlling, and preventing patient illness and disease within the assigned public health nursing program;

to perform physical examinations; identify patients with physical and emotional illness; and to administer medications as prescribed.

Responsibilities & Duties

Contributes to physician's effectiveness by identifying short-term and long-range patient care issues that must be addressed;

providing information and commentary pertinent to deliberations; recommending options and courses of action; implementing physician directives.

Assesses patient health by interviewing patients; performing physical examinations; obtaining, updating, and studying medical histories.

Determines abnormal conditions by administering or ordering diagnostic tests, such as x-rays, electrocardiograms, and laboratory studies;

interpreting test results.

Documents patient care services by charting in patient and department records.

Instructs and counsels patients by describing therapeutic regimens; promoting wellness and health maintenance.

Provides continuity of care by developing and implementing patient management plans.

Maintains safe and clean working environment by complying with procedures, rules, and regulations.

Protects patients and employees by adhering to infection-control policies and protocols.

Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation;

enforcing adherence to requirements; advising management on needed actions.

Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks;

participating in professional societies.

Develops health care team staff by providing information, educational opportunities, and experiential growth opportunities.

Contributes to team effort by accomplishing related results as needed.

Position Summary :

The Alignment Jump Start Assessment program is a physician-led and advanced practice clinician-driven program that is intended to care for and support Alignment Healthcare patients with complex and chronic care needs.

As a JSA Advanced Practice Clinician, you will work within an interdisciplinary team environment that includes physicians, nurse practitioners, physician assistants, case managers, and other health care team members to ensure proper delivery of clinical and home-based patient care.

In this role, you will assess, develop, and coordinate options and services promoting quality care in order to achieve optimal health care outcomes while also ensuring cost-effective care complying with Alignment policy and all state and federal regulations.

Our members are typically aged 65 and over and home and virtual visits will be between 45 and 60 minutes depending on whether it is an annual or initial visit.

General Duties / Responsibilities :

May include but are not limited to)

JSA APC’s responsibilities

Conduct initial or annual face-to-face patient visits in home and clinic settings

Follow CMS guidelines to make virtual visits when appropriate)

  • Health Risk Assessment includes : Past medical history Review of symptoms Physical examination Medication review Cognitive / depression screenings Fall assessment / functional assessment Immunizations and preventive health screenings including Colonoscopy, Breast Cancer screening, Bone density, and diabetes eye exams Advance Care Planning and POLST
  • Update and reevaluate the year-to-year diagnoses following CMS guidelines and HCC coding
  • Identify diagnoses and develops proper care plan for chronic disease management
  • Provide patient / family / caretaker education and facilitate patient empowerment and quality of life
  • Matching care desired with care given while providing true, informed consent
  • Collaborate with HEDIS / STARS and other Alignment programs and departments to facilitate optimal program integration and continuity of care
  • Refer patient to an appropriate program such as Care Anywhere Program
  • Refer patient to social worker for appropriate community resources as needed
  • Provide contact information for patient to receive medical assistance to avoid unnecessary emergency room visit and hospitalization
  • Comply with all HIPAA regulations and maintains security of protected health information (PHI)
  • Complete EMR documentation with quality that meets Alignment expectations in a timely manner.
