Job Summary and Qualifications

Perform direct patient care within the scope of the Physical Therapy Practice Act, with emphasis on processes and patient outcomes.

What you will do in this role

  • Perform comprehensive, age specific patient assessments for appropriate care setting : Acute care, Inpatient Rehab, and / or Outpatient
  • Provide individualized, goal-directed care.
  • Formulate a plan of care based on patient diagnosis and identified therapy related problems.
  • Develop and implement teaching plans for patients concerning promotion, maintenance and restoration of patient function and independence.
  • Provide ongoing education, training, and emotional support to foster a partnership with the patient, family and significant others.
  • Provide age specific care from neonate to geriatric in the appropriate care setting : Acute care, Inpatient Rehab, and / or Outpatient
  • Evaluate the patient's (individual and family) responses and outcomes to therapeutic interventions.
  • Use critical thinking approach to analyze clinical data and current literature as a basis for decision making in therapy practice.
  • Communicate pertinent payor needs / requirements to patient, patient family, treatment team, and physician.
  • Make assignments to staff and delegate to unlicensed staff in compliance with current Physical Therapy rules and regulations and department operations.
  • Assume accountability and responsibility for the quality, continuity, and coordination of therapy across the care settings.
  • Act as patient advocate to implement and direct therapy care to ensure patient safety in the clinical practice environment.
  • Integrate the management of information and effective communication skills in daily practice.
  • Serve as a healthcare advocate in monitoring and promoting quality health care and services for the patients served.
  • Participate in activities and act as a leader in promoting best practices, department specific initiatives, customer satisfaction, and process improvement.
  • Demonstrate high ethical standards in customer service through all interactions with the patients, families, coworkers, staff, other hospital departments, physicians, community and all others.
  • Create a high quality, professional clinical environment to facilitate the necessary recruitment of appropriate numbers of qualified staff.
  • Support the Mission, Vision, and Values of the Organization.
  • Other duties as assigned.

What qualifications you will need :

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date
  • Licensed Physical Therapy
  • Bachelors Degree

Responsibilities

Position Summary

Evaluates all assigned patients and develops, implements and modifies treatment plan in accordance with the patient’s need and physician direction.

Responsible for delegation and direction of all discipline-specific therapy services for those assigned patients. Ensures the design of functional treatment programs allowing for an effective return to community activities.

Serves as an educator of patients, families, students, co-workers and self. Performs other professional duties associated with the care of the patient, development of discipline-specific therapy services and design of program delivery.

Specific Duties

  • Evaluates assigned patients and establishes appropriate treatment goals and treatment programs.
  • Provides ongoing evaluation of patient’s status and goals, modifying treatment program for timely progression.
  • Develops comprehensive goals for patient treatment to maximize patient outcomes and return to community activities.
  • Coordinates treatment program in conjunction with other clinical / therapeutic services, as well as available community resources.

Attends appropriate clinical meetings to facilitate coordination.

  • Provides for patient education to include the development of comprehensive and progressive home programs with instruction to the patient, family and other caregivers as appropriate to facilitate a safe and effective discharge.
  • Prepares for patient discharge inclusive of ordering of necessary equipment and recommendations for therapeutic services ensuring the continuum of care.
  • Evaluates patient in the actual or simulated home, school, work or community environments as necessary to facilitate a return to community activities, making recommendations for necessary adaptations or training.
  • Identifies and manages various levels of care necessary to ensure care plan delivery. Delegates and supervises the utilization of PTAs, as well as support personnel to maximize patient outcomes and ensure effective utilization of services.

Qualifications

Minimum Qualifications

  • Current state licensure or eligibility for state licensure as a Physical Therapist required.
  • Certified BLS or completion in the first 90 days of employment required.
