PT Job Summary :
The Physical Therapist provides quality care to all residents. The PT evaluates and treats patients / residents, communicates with their families, physicians, and other health team members, and maintains all necessary documentation of services in the patient’s medical records.
Physical therapy services including treatment planning and implementation, discharge planning, testing patients’ physical abilities, analyzing rehabilitation goals, and consulting with the rehab team and other health team members to coordinate the most effective treatment program.
Responsibilities and Duties :
- Evaluate patients promptly upon receiving physician referral for treatment.
- Develop effective treatment plans.
- Obtain approval for services from referring physician.
- Treat patients according to treatment plan approved by physician.
- Record evaluations, daily treatment notes, weekly progress notes, 14-day progress report to the physician, recertifications, and discharge summaries per Company and facility policy and procedure
- Participate in discharge planning.
- Secure necessary durable medical equipment (e.g., orthoses, walkers, canes elevated toilet seats, and grab bars) for patients to facilitate independent mobility.
- Submit all billing logs and necessary information on a timely basis.
- Supervise up to two Physical Therapist Assistants.
- Supervise students during the internship.
- Provide clearly defined, career-related (i.e., non-clerical) responsibilities and tasks in accordance with the students’ academic programs.
- Inform student interns about ensuring their compliance with company, facility, state, and federal guidelines.
Other requirements :
- May be entry level or experienced.
- Must have strong computer skills, including proficiency with Microsoft Office.
- Must have excellent interpersonal skills.
- Must exhibit an elevated level of confidentiality.
Qualifications :
Licensed as a Physical Therapist in the State of California.
- Must have valid CPR certification.
- Must have a completed physical examination from a licensed physician.
- Must have a negative TB skin test within the past year OR a clear Chest X-ray within the past five years.

Physical Therapist ()
Responsibilities
1. Review available resident information and evaluate the impact of this information on the assessment and treatment process.
2. Conduct and document a thorough functional assessment of the resident, utilizing standardized procedures. Assessment of home / vocational and equipment needs.
3. Develop and implement an individualized treatment plan based on resident needs and containing achievable functional goals.
4. Comply with evaluation, treatment and documentation guidelines and consistently maintain timely and accurate department records.
5. Instruct Assistants in treatment procedures to be utilized and provide close supervision while such staff members are working with residents.
6. Enter a progress note in the resident record following each treatment session.
7. Revise treatment plans, as needed, throughout the treatment process. Physical Therapist
8. Document the course of treatment in a clear concise discharge summary utilizing a prescribed format.
9. Orient residents to program services and treatment procedures appropriate to their needs. Inform residents of any potential risk during any procedures.

11. Instruct residents and family / other caregiver about exercise programs and therapeutic procedures to be continued by the resident at home following treatment.

14. Attend and participate in other department / facility meetings, as required.
15. Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical competency.
16. Participate in the quality improvement process.
17. Promote the programs and services of Heritage Healthcare through formal and informal interactions with the community.
18. Act in compliance with Heritage Healthcare’s regulatory and professional standards and guidelines.
19. Provide input to the Rehab Manager on clinical performance of assistants, as requested and appropriate.

Qualifications
1. Bachelor / Masters of Science degree in Physical Therapy.
2. Current license in the state of practice.

5. Excellent oral and written communication and interpersonal skills.

9. Ability to complete the following activities safely, and at times, for prolonged periods :
- Standing : Maintaining an upright position on the feet, especially for prolonged periods of time
- Pushing : Applying pressure and exerting force against an object for the purpose of moving it forward or away from the source of the force push or pull force up to 35 lbs.
- Lifting : Raising an object from a lower to a higher position, or moving an object laterally, from position to position
- Required to lift up to 50 lbs of force independently. May be required to lift over 50 lbs with assistance of another person or a mechanical lift
- Stooping : Bending the body forward and down from the waist, hips and knees, sometimes for a prolonged length of time.

Physical Therapist
ECH : Physical Therapy IP (Full-Time, Day Shift)-

The Physical Therapist assesses, plans, implements prescribed physical therapy treatments and supervises individual patient care.
Provides quality services autonomously in a Hospital setting. Assists patients to prevent disability and aims toward the promotion of maximal patient / caregiver participation.
Is responsible for recovery program following disease, injury, surgery or loss of a body part. Provides care according to Physical Therapy, Medical Center policies and procedures and the application of treatment techniques in accordance with discipline specific designated practice act.
Provides in-patients services under physician's orders. Supervises Physical Therapy Assistant upon request. Participates in case conferences and discharge planning meetings and coordinates with Case Manager for the patient who may continue to require Physical Therapy services after hospital discharge.
These services may be provided for both LLUMC and LLUCH based upon adult or pediatric assignment. Performs other duties as needed.
Graduate from an accredited Physical Therapy program required. If assigned to NICU departments, minimum two years of post-graduate pediatric experience required.
Currently licensed by the Physical Therapy Board of California and California Children's Services (CCS) paneled if work on Pediatric Rehabilitation or CCS paneling within twelve months hired in to Pediatric Program.
Current Basic Life Support (BLS) Certification required.
- Demonstrates the ability to meet Department documentation standards and regulation requirements. Able to read; write legibly;
- speak in English (and Spanish preferred) with professional quality; use computer and software programs necessary to the position;
- troubleshoot and calibrate patient care equipment. Able to relate and communicate positively, effectively, and professionally with others;
