The schedule for this position will be Sunday through Thursday (10am to 7pm)

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Perform mental health screening of potential patients and make recommendations for admission acceptance or rejection.
  • Maintain a clinical caseload.
  • Recommend follow-up treatment for new admissions with Axis I or II diagnosis.
  • Provide individual, group, and family therapy, and other clinical services as requested.
  • Maintain current, accurate and confidential clinical documentation and submit reports in a timely manner.
  • Hold clients accountable when necessary for inappropriate or negative behaviors, and utilize effective confrontation techniques and behavior interventions to help improve behavior.
  • Ensure debriefing with staff at the beginning and end of each shift with the oncoming staff.
  • Ensure immediate communication with the Clinical Director on outstanding incidents involving absconders, physical violence, suicidal threats, physical accidents involving staff or clients, and extreme negative behavior.
  • Adhere to all Phoenix House policies and procedures.
  • Ensure program quality and integrity through effective compliance with contracting and licensing agency established guidelines.
  • Participate in staff meetings and training exercises as directed.
  • Represent Phoenix House in court, interagency meetings, and community affairs as directed.

  • Ability to maintain a good working relationship with all co-workers and the general public and to use good judgment in recognizing scope of authority.
  • Ability to remain calm in stressful situations.
  • Ability to take a teamwork approach to the job by cooperating with others, offering to help others when needed, and considering larger organization or team goals rather than individual concerns.

This includes the ability to help build a constructive team spirit where team members are committed to the goals and objectives of the team.

  • Must be computer-literate with working knowledge of Microsoft Office programs.
  • Ability to operate general office equipment in the completion of job responsibilities.
  • Demonstrate proficiency in Phoenix House’s electronic health record system (Welligent) sufficient to fulfill the duties required by this position within 30 days of employment.

EDUCATION / EXPERIENCE : Must possess a Master’s degree in related field from an accredited college or university. A minimum of two years experience in a substance abuse or mental health setting is required.

LICENSES / CERTIFICATIONS / CEUs :

A current, valid State of Florida license as a Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW) is required.

A valid State of as a Certified Addictions Professional (CAP) is preferred. A valid Florida driver’s license is required.

Florida Certification

This position is classified as direct service. Employee must maintain required continuing education specifically related to any certification or license held, Phoenix House requirements, and regulatory and licensing agency requirements.

We are seeking Case Manager / Mental Health Counselor for The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV / AIDS.

The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being.

Seventy-four units will be allocated for residents referred by the New York City HIV / AIDS Services Administration (HASA) and forty-nine apartments will be for families referred by the New York City Housing Authority (NYCHA).

The Case Manager / Mental Health Counselor is responsible for providing supportive case management services to the HASA-funded residents.

The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV / AIDS population.

The Ideal Candidate :

  • You have a Master’s Degree in Social Work or a related field required

  • You have at least 5 years of experience in the mental health field
  • You have a Credentialed Alcohol and Substance Abuse Counselor (CASAC) certification is a plus

  • You have solid computer skills internet, Microsoft Office, and charting database

Key Responsibilities :

  • Maintain a caseload and provide regular case management services to these individuals
  • Maintain resident records in compliance with agency and funding standards
  • Conduct and review assessments
  • Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency
  • Monitor and ensure residents are linked to healthcare providers
  • Provide counseling and crisis intervention as necessary
  • Assist residents in securing and maintaining benefits
  • Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs
  • Facilitate groups and activities that promote health and independent living skills development
  • Monitor and assist residents with rent payments and budgeting
  • Conduct regular unit inspections
  • Educate residents about approaches to mental health
  • Compile resources on mental health information and treatment strategies
  • Attend ongoing training to enhance the services provided to the residents
