ABOUT THE SENIOR MEDICAL ASSISTANT
Upperline Health Plus is seeking an experienced Medical Assistant to support a team of clinicians in our Tampa, FL clinic delivering complex plans of care in accordance with the patient’s health status and overall goals and values.
In addition to clinical support, this individual will be providing care coordination, including all telephonic and back-office support required by the team of clinicians.
In this role, you get to act as a patient’s guide to a complex and confusing health care system. You will provide critical support activities to coordinate personalized care delivery for Upperline Plus patients and patients’ families.
Ideal candidates will have experience identifying and coordinating the needs of patients with multiple chronic conditions as well as supporting clinicians to help them operate as efficiently as possible.

WHAT YOU’LL DO
- Build relationships with Upperline’s geriatric and chronically ill patients during their visit to our podiatry clinic and inform them of our Upperline Plus program, which provides preventative and coordinated care services
- Conduct health assessments and complete visits with Upperline Plus patients being seen in our Podiatry Clinics
- Complete vitals and required documentation in clinic EMR (AthenaHealth)
- Provide routine foot care for at-risk patients with Diabetes and / or other chronic illness
- Facilitate and support virtual provider encounters in the clinic
- Telephonic outreach and proactive engagement with Upperline Plus patients to coordinate ongoing preventative care
- Care coordination activities as assigned by Upperline Plus Nurse Practitioner including, but not limited to, medical records requests, order management, appointment scheduling, managing patient care referrals, etc.
- Identifying community resources and assisting Upperline’s patients and families in accessing those resources
- Participation in weekly interdisciplinary team meetings
- Other related duties as deemed necessary by Upperline Health
QUALIFICATIONS OF THE SENIOR MEDICAL ASSISTANT
- A minimum of 2 years’ experience as a medical assistant providing direct patient care is required
- Previous experience working with adult geriatric or chronically ill patient populations is highly preferred
- Experience in an outpatient clinic setting in a patient facing role is highly preferred
- Clinical care coordination or care management experience is ideal
- Prior wound care and foot care experience is a plus
- Bilingual English / Spanish is helpful
- Previous experience working in an EMR system ideally, AthenaHealth
- Database management skills and proficiency using Word, Excel and other Microsoft Office products is necessary
- Extremely strong customer-service orientation and work ethic
- Exceptional organizational skills and ability to multi-task
- Self-starter with an entrepreneurial spirit and demonstrated problem solving skills

Your Specific Responsibilities :

Verifies and updates demographic, contact and financial information at each visit to ensure accuracy.
- Verifies appointments and checks clients into EHR, retrieves, files and assembles medical records as necessary, knowledge of service collection and use of service collection tools / documentation.
- Knowledge of financial and billing procedures from all sources (Medicaid, Medicare, managed care plans, Ryan White, etc.

When assigned to clinical duties (including specialized clinics) :
Updates EHR as necessary and prepares clients to be sign by provider. Takes vital signs, height and weight and documents adherence questions and updates EHR accordingly.
Informs providers of any known abnormal lab or diagnostic results and informs provider when patients are ready to be seen.
Prepares and maintains exam rooms and ensures appropriate supplies I equipment items are available to ensure clinical service delivery can be provided.
Cleans and / or sterilizes equipment, maintains inventory of supplies, rotates stock, and monitors expiration dates.
High level of expertise related to inputting, processing and retrieving lab orders, results. This also includes expertise in preparing lab orders for patients who receive lab services at designated patient service centers.
Knowledge of medical insurances and contracted labs associated with medical insurance.

- Arrives on time and ready at workstation at assigned time. Informs supervisors of changes in schedule.

Departs from clinic those patients that do not require nursing intervention. Maintains and uses principles of aseptic techniques and universal precautions when performing clinical duties and tasks.
Maintain the knowledge and skill necessary to provide care or services based on physical, psychosocial, educational, safety, and related criteria appropriate to the age of the patients served in the assigned service area.
Utilizes computerized systems according to departmental needs.
- Provide support to the provider during clinic by assisting with phone calls and faxes.
- Recognize emergency situations and implements emergency procedures. Administers first aid and CPR.
