Position Overview
HELP USA is seeking a Case Manager to provide services and referrals to homeless veterans who reside in HELP USA transitional housing under the Veteran Administration Grant per Diem Program.
As a Case Manager, your responsibilities will include :
Conducting initial intake interviews and assessments and developing an independent living plan.
Providing advocacy for public benefits including VA benefits, Public Assistance, Social Security, etc.
Identifying barriers and challenges that veterans face and utilizing a solution-based approach to effectively resolve issues that threaten housing stability.
Preparing housing stability plans with a path to permanent housing and self-sufficiency.
Serving as liaison with appropriate agencies and programs and maintaining a network of resources and contacts
Requirements :
Bachelor’s Degree in Social Work or a related field preferred.
Knowledge of the challenges faced by the homeless veteran population and the supportive services available to successfully achieve the goal of permanent housing and self-sufficiency.
Prior experiences in case management, assessment, counseling, crisis intervention and housing placement preferred.
Knowledge and understanding of team concepts preferred.
Valid US driver’s license preferred.
Computer literate specifically with Microsoft applications required.
Compensation : $17.79 hourly

Join the team of highly dedicated mental health professionals. We are presently looking for a Case Manager who will be responsible for assisting the treatment team by facilitating the treatment process, by reviewing and being familiar with initial team assessments and treatment plans, along with being familiar with patient goals and needs.
The Case Manager documents patient goals and progress toward goals, utilizing recommendations and consultations of the treatment team.
The Case Manager will make necessary contacts to carry out the discharge plan.
ESSENTIAL FUNCTIONS :
- Provide case management and care coordination services for clients / patients / residents in the facility.
- Responsible for coordinating continuum of care activities for assigned clients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements.
- Conduct various duties related to coordinating treatment and discharge activities, as needed.
- Provide quality care through developing, implementing, managing and evaluating client / patient care plans.
- Collect, correlate and provide clinical data to the treatment team.
- Coordinate in a timely manner, issues or activities relevant to the treatment team.
EDUCATION / EXPERIENCE / SKILL REQUIREMENTS :

Well Care Services is a Behavioral Health Clinic looking for Case Managers to join our team. We are hiring a team of case managers that will be working hands on with patients out on the field.
Requirements :
Bachelor's degree in Human Services or related field and a minimum of two years of experience working with adults with serious mental illness;
or an equivalent combination of training and experience. Knowledge of serious mental illness, substance abuse, psychotropic drugs, entitlement programs and recovery oriented services required.
Interviewing, service planning, consultation, public relations, and experience with electronic medical record. Computer skills and the ability to type required.
Good driving record required. Must not reflect a total accumulation of six or more negative points within the past three years or a major violation within the past four years.
Pre-employment drug testing and FBI criminal background check required.
