Case Manager needed on Staten Island
Full-time position, 35 hours / week!
NOTE : This is a full on-site posiiton.
POSITION OVERVIEW :
The Patient Navigator / Case Manager acts as a link between clients and the range of services available through the CCBHC.
S / he will engage individuals with complex behavioral health needs to develop person-centered care plans, provide care management services, track and arrange appointments, educate clients and coordinate other aspects of clients' health and community services.
KEY ESSENTIAL FUNCTIONS
- Work collaboratively with clients, social workers, and medical / psychiatric providers to provide clients with rapid access to evaluations and services
- Help clients navigate the intake process and, as appropriate, assist clients in understanding their diagnosis, treatment options and resources available
- Provide outreach services to clients to increase engagement in services
- Help clients address barriers to care
- Strong advocacy skills including ability to work collaboratively with internal and external providers
- Provide resources and referrals to clients as appropriate
- Ensure that documentation is completed within required time frames in required systems
- Participate in staff development and training activities
- Perform other responsibilities and duties as assigned
REQUIREMENTS :
- Bachelor's Degree in human service or public health field required.
- Strong interpersonal, organizational, problem solving, and communication skills.
- Must demonstrate initiative, creativity, flexibility, and basic computer skills.
- An appreciation of a mental health and social service environment and a commitment to providing person centered care
- Ability to work some evening and weekend hours required.

About ORR : The Office of Refugee Resettlement (ORR) is a program of the federal Administration for Children and Families (ACF) within the United States Department of Health and Human Services.
ORR programs provide short term home based transitional foster care and residential services for unaccompanied minors who immigrate to the United States without a parent or guardian.
Job Summary : The case manager will provide quality case management services and support to a caseload of eight unaccompanied children in Sheltering Arms’ in care.
Essential Functions
- Meet all the mandates of the Office of Refugee Resettlement and OCFS for case management services to unaccompanied children in the care of Sheltering Arms
- Interview children to assess their needs, immigration journey, health history and all required psychosocial information
- Develop, Implement and Carry out preliminary services plans, individualized service plans and service plan reviews in the required timeframes
- Locate, screen and assess children’s potential sponsors
- Ensure children are released to sponsors within 30 days of intake
- Maintain communication and coordination with each child’s sponsor and family about the child’s needs and well being
- Complete progress notes and maintain case records according to ORR contractual obligation and OCFS standards.
- Review cases regularly with the Lead Case Manager
- Collaborate with foster parents, nurses and social workers to meet health and mental health needs
- Coordinate planning with immigration, customs, legal representatives, homeland security and child advocates
- Advocate for the children and agency in Immigration court
- Attend required annual training and supplemental training to keep current with new developments and resources
- Conduct home visits
- Performs other duties as delegated
Qualifications :
- BSW / BA in related field
- Strong writing, relationship and organizational skills
- Bilingual in Spanish or Haitian creole required
- Must be able to work evenings and weekends
