Case Manager.
Summary
This position has the primary responsibility of providing therapeutic program services to resident offenders. It is assigned a specific caseload of resident offenders and monitors the service delivery for them while they are enrolled in the program.
These services may include didactic, psycho- educational, individual and group counseling, peer awareness activities, therapeutic encounters, crisis intervention and recordkeeping.
The Counselor / Case Manager provides these services within the scope of local certification, licensure and credentialing authorities and in accordance with internal and external assurances and prohibitions.
Primary Duties and Responsibilities
- Ensures compliance with standard internal and external and other accrediting agency contract policies and procedures.
- Facilitates daily clinical program activities in accordance with the established clinical schedule. Activities may include daily process groups and therapeutic community (TC) meetings, weekly program curriculum lectures, Life Skills seminars, caseload groups and specialized groups focusing on the areas of Anger Management, Relapse Prevention, Job Readiness preparation and Parenting Skills.
- Conducts initial intake session with new resident offenders assigned to the program and orientate them about program requirements, reviews the inmate’s criminal file and establishes a preliminary treatment plan.
- Conducts bi-weekly individual counseling sessions with assigned caseload. These sessions may include the following tasks : completion of a monthly progress report, monthly treatment planning, goal-setting, crisis intervention, behavioral adjustment sessions, psychological referrals, and discharge planning in order to provide recommendations and referral sites for continued treatment upon their release
- Maintains clinical charts (electronic and hard copy versions) for assigned resident offender caseload and ensures all documentation is completed in accordance with established deadlines and program policies.
- Monitors and addresses behavioral issues displayed by the resident offender population and conducts daily visual room inspections.
- Participates in compilation of data and program reporting functions.
- Participates in staff meetings, seminars and workshops, as required.
- Performs other duties as assigned.
Job Requirements
Minimum Requirements
Bachelor’s degree with concentration in the field of Psychology, Criminal Justice,
Counseling, Social Work, or related field(s) and / or a Certified Alcohol and Drug Counselor (CADC). Master’s degree preferred.
Preferred licensures and / or certifications as a CADC, LCADC, LPC, LSW, and LCSW.
Knowledge of the fields of addictions or mental health counseling preferred. Required skills include : time management and organizational skills.
Ability to adhere to strict deadlines.
Good written and oral communication skills.
Ability to work with computers and the necessary software typically used by the department.

About ORR : The Office of Refugee Resettlement (ORR) is a program of the federal Administration for Children and Families (ACF) within the United States Department of Health and Human Services.
ORR programs provide short term home based transitional foster care and residential services for unaccompanied minors who immigrate to the United States without a parent or guardian.
Job Summary : The case manager will provide quality case management services and support to a caseload of eight unaccompanied children in Sheltering Arms’ in care.
Essential Functions
- Meet all the mandates of the Office of Refugee Resettlement and OCFS for case management services to unaccompanied children in the care of Sheltering Arms
- Interview children to assess their needs, immigration journey, health history and all required psychosocial information
- Develop, Implement and Carry out preliminary services plans, individualized service plans and service plan reviews in the required timeframes
- Locate, screen and assess children’s potential sponsors
- Ensure children are released to sponsors within 30 days of intake
- Maintain communication and coordination with each child’s sponsor and family about the child’s needs and well being
- Complete progress notes and maintain case records according to ORR contractual obligation and OCFS standards.
- Review cases regularly with the Lead Case Manager
- Collaborate with foster parents, nurses and social workers to meet health and mental health needs
- Coordinate planning with immigration, customs, legal representatives, homeland security and child advocates
- Advocate for the children and agency in Immigration court
- Attend required annual training and supplemental training to keep current with new developments and resources
- Conduct home visits
- Performs other duties as delegated
Qualifications :
- BSW / BA in related field
- Strong writing, relationship and organizational skills
- Bilingual in Spanish or Haitian creole required
- Must be able to work evenings and weekends
