Position Summary

Career Interest : Responsible for providing clinical social work services to patients and families and acting as an advocate to help clients to access resources while confronting their personal issues such as mental illness, addiction and abuse.

Provides individual psychotherapy, family therapy and group therapy.

Responsibilities :

• Assesses patient and / or family biopsychosocial needs and develops treatment plan in concert with physicians and other providers
• Provides individual psychotherapy, family therapy and group therapy utilizing evidenced based therapies to address psychiatric illnesses
• Documents patient / family status and history, diagnosis, treatment plan, evaluation results, observations, progress in medical record
• Serves as a resource to other staff on community resources in area of specialty
• Serves as patient advocate and liaison with physicians, families, and community to provide quality care and address client needs
• Provides clients with community resource information
• Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

As a hospital social worker you will assist our patients in dealing with the effects of hospitalization. Patients and families are often under a lot of emotional pressure and financial stress due to illness or injury.

Additionally, there are needs that need to be addressed after discharge such as home care or rehab services. You will help our patients and their families navigate the process of illness and injury, from hospitalization to recovery.

Responsibilities :

• The Social Worker will monitor the patient referral throughout the hospital stay to ensure the appropriate environment of care.
• The role of the Social Worker is to provide supportive counseling to patients in accordance with state and federal standards of care and Managed Care partners.

Starting Pay

Our pay ranges are market competitive. The pay range for this job begins at $22.13 / hr, but your offer will be determined based on your education and experience.
