Job Summary and Qualifications
As a Medical Social Worker, you will be responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated.
You will act as a liaison between the patient and community resources / post-acute care providers. You will create and facilitate timely implementation of discharge plans and arrange post-acute interventions identified in the plan of care.
You will facilitate patient movement across the continuum of care and identify, address, and track barriers to patient throughput.
- You will partner with the RN Case Manager to ensure the post-acute medical needs and level of care are appropriate.
- You will respond promptly to referrals from the RN Case Manager or other staff when risk factors for social determinants of health are identified.
- You will perform thorough assessments and develop appropriate plans of care for high risk patients with discharge planning or psychosocial needs.
- You will perform proactive discharge planning.
- You will provide expertise in post-acute services and community resources.
- You will identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical and spiritual.
- You will provide psychosocial support and resources.
Experience

- Bachelor's Degree in Social Work

Licenses and Certifications

Description
Role Mission : Social Workers work directly with our at-risk scholars to ensure that they graduate from IDEA and go to and through college by providing them individualized emotional, behavioral, and mental health support.
Social Workers manage a caseload of at-risk scholars and provide individual and group interventions to support them independently master coping skills to address and overcome mental, behavioral, and academic challenges.
Social workers also support students and families with the removal of barriers to persistence, attendance, and academic success through connections to community resources.
This is an ESSER III Grant, grant funded position and funding may be extended. A grant-funded position has no guarantee of ongoing and indefinite employment.
All duties and responsibilities tied to this position are allowable and allocable to the grant.

Compensation :
Salaries for people entering this role typically fall between $58,180 and $64,812, commensurate with relevant experience and qualifications and in alignment with internal equity.
This role is also eligible for a performance bonus based on team performance and goal attainment.

Responsibilities

Qualifications
