Position Highlights

Collaborate with the interdisciplinary team to promote and protect resident rights and the well-being of all patients / residents.

This includes educating staff on the role of the social services

  • Participate in the development of a written, interdisciplinary plan of care for each patient / resident to address social services needs
  • Hold meetings with patients / residents, families, and significant others as requested and to provide support and information so as to improve quality of life
  • Perform patient / resident discharge planning activities such as family / patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
  • Establish relationships and maintain contact and referral flow with community based agencies / services

  • Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and / or Substance Abuse is preferred.
