• Advanced Certified Hospice & Palliative Social Worker (ACHP-SW) certification or ability to take exam within 24 months of being hired.

• Minimum of two (2) years working in a direct clinical / therapeutic capacity
• Minimum of two (2) years working with life-threating conditions, chronic disease, palliative care and / or hospice, end-of-life care, and related situations
• Knowledgeable of a full range of specialized and expert clinical social work / counseling services for patients / families receiving palliative care
• Able to provide crisis intervention, short-term and family therapy; Offers information and referral and / or tangible assistance
• Organizational and prioritization skills while determining severity of patient condition and prognosis
• Excellent verbal and written communication skills
• Basic computer skills

JOB TITLE ( # 1247 ) : Clinical Social Worker Health Care Facility Surveyor - Georgia

CMS’s Long-Term Care Basic Training and SMQT certification are required

Impact Recruiting Solutions is currently seeking a Clinical Social Worker Health Care Facility Surveyor to fill an opening with a Quality Improvement Consulting Company and will work in a technically exciting environment supporting internal and external customers nationwide.

Requirements of the Clinical Social Worker Health Care Facility Surveyor :

• Must have an Associate or Bachelor's degree in Social Work
• Must have successfully completed CMS’s Long-Term Care Basic Training and passed the Surveyor Minimum Qualifications Test (SMQT).
• Must have at least two (2) years of recent experience working as a surveyor accrediting or certifying facilities that serve the residents of long-term care facilities.
• Must maintain current licensure to practice as an LCSW
• Demonstrated history of independent decision-making skills to direct and effectively manage the survey process.
• Ability to set priorities independently and collectively in performing survey tasks.
• Ability to openly discuss conflicts / controversy, and to seek assistance when appropriate to make decisions and resolve conflicts.
• U.S. Citizenship is required for this position
• Ability to travel up to 75% of the time on a regular basis is required

JOB PURPOSE : The Social Worker strives to promote patient and family wellness, improved care

outcomes, access to appropriate hospital and community resources, and manages, supports and

develops comprehensive transitional care plans for patients with complex and psychosocial needs in

Inpatient and Emergency Department (ED) environments.

MINIMUM EDUCATION REQUIRED : Master’s degree from an accredited social work program required

MINIMUM EXPERIENCE REQUIRED : None

MINIMUM LICENSURE / CERTIFICATION REQUIRED BY LAW : None

ADDITIONAL QUALIFICATIONS : Prefer a minimum of two (2) years of experience in an acute or postacute setting. Licensed master social worker (LMSW) in state of Georgia preferred.
