Responsibilities
The Social Worker (MSW) in Home Health assists patients and families in coping with problems resulting from severe or long-term illness, and with difficulties in recovery and rehabilitation.
The Social Worker (MSW) will assess, diagnose, and treat patient's mental and social conditions, counsel individuals and or families, and update case records.
The licensed social worker with a master's degree performs these functions under the supervision of the licensed master's degreed social worker.
- Instructs health care team members on community resources available to assist patients.
- Plans for continuity of care with hospitals and community agencies.
- Assesses and treats social and emotional factors related to patient's illness to determine ability to cope with daily living problems.
- Assists the patient and significant others to understand, accept, follow, and implement medical recommendations.
- Assists the patient and significant others in utilizing community resources which will help the patient to achieve and maintain optimal functioning.
Education and Experience
Education Requirements
- Master's Degree from a school of Social Work accredited by the Council of Social Work Education.
- One year of social work experience in a healthcare setting.
License Requirements
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Clinical Social Worker
Monday through Friday position with no on-call or weekend hours required. For the first few months this could be a part-time position

InnovAge PACE (Program of All-inclusive Care for the Elderly) is an alternative to nursing facilities. It provides customized healthcare, social engagement, and exercise to older adults to help them stay independent and in their homes.
PACE participants receive most of their healthcare and social support at an InnovAge center in their community. Each person in the program has a team of healthcare experts and a personal care plan that is focused on providing a personalized experience.
Our Behavioral Healthcare Provider (BHP) is an integral part of developing and delivering a personal care plan ensuring high quality, clinically appropriate, team-based behavioral health care.
Provides initial assessments, clinical intervention, referrals for participants and families, and treatment recommendations.
Physical attendance at an InnovAge center is essential to performing the job duties with the ability to travel to meet participants outside of center as needed.
You will
- Confidently navigate ambiguous situations
- Multitask, be flexible, and comfortable in transforming environment
- Adapt interpersonally to meet participant and Interdisciplinary Team (IDT) clinical needs in real time with insight into personal strengths
- Familiarity with Motivational Interviewing, Solution-Focused Brief Therapy, Dialectal Behavioral Therapy, Acceptance Commitment Therapy, and Cognitive Behavioral Therapy
- Be open to feedback and able to reflect productively
Clinical Care 70%
- Completes mental health assessments and document in the EMR (diagnosis, recommendations, team-based care planning, and other regulatory requirements)
- Provides solution-focused (monitoring, therapeutic, or crisis) intervention to participant and / or caregiver(s)
- Consults with team members regarding clinical findings, provide interventions, and case conferencing
- Collaborates with IDT / external providers / caregivers to support participants at risk for inpatient psychiatric hospitalization (e.
g., referral and coordination with outside resources to promote stabilization)
Monitors response to psychotropic medications and coordinate with psychiatric consultant to ensure continuity of care
Administration 30%
- Educates IDT to manage and resolve behavioral complexities or placements
- Liaisons with external psychiatric providers to ensure appropriate referrals, continuity of care, monitoring / follow up and report back to IDT
- Participates in IDT meetings, conferences, committees, and events as requested
REQUIRED
- 1+ year working in an integrated care setting
- Licensed Clinical Social Worker (LCSW)
- Current CPR certification or the ability to obtain CPR certification in first 30 days of employment
PREFERRED
- Knowledge working with geropsychiatric populations
- Training or experience with group and family interventions
- Experience with conflict management, mediation, de-escalation, and crisis mitigation
- Knowledge of the chronic disease management and its psycho-social implications
- Bi-lingual preferred based on location
- A doctoral degree in clinical psychology or integrated primary care experience
- Licensure or certification in substance use treatment
- Licensed as Psychologist

Position Summary
- The Social Worker plans, organizes, develops, and implements Social Services programs ensure all medically-related emotional and social needs of patients are met in accordance with all applicable laws, regulations, and Life Care standards.

Currently registered / licensed in applicable State (if required by State law). Must maintain an active license in good standing throughout employment.

- Bachelor’s degree in a human services field (which may include gerontology) if working in a facility with 120 or more beds (see State law)

Specific Requirements

Social Worker
The Social Worker is that member of the core interdisciplinary group whose primary function is to perform an assessment of psychosocial symptoms of the patient and family, develop psychosocial outcomes for alleviating identified symptoms and help provide the psychoeducation, supportive counseling and casework interventions needed to meet these outcomes.
The Social Worker also provides psychosocial education and consultation to the IDG.
- Participates in the preparation, evaluation and execution of the interdisciplinary plan of care to assure that the highest quality psychosocial care is provided to patients / families and that continuity and comprehensiveness of care are
- Performs bio-psycho-social-spiritual assessments of patient / families, rather than diagnostic

JOB REQUIREMENTS
- Ability to travel per job requirements.
- Thorough knowledge of community resources or the desire / ability to obtain this knowledge.
- Knowledge of key psychosocial issues prevalent with terminally ill patients and their family / caregivers, including the knowledge of effective psychosocial inter mentions to address those issues.
- Capacity to relate and work well with others as a member of an interdisciplinary team.
- Excellent listening and communication skills.
- Ability to be an effective patient and family advocate- both within the patient / family unit, within the core IDG, and within community agencies and the like.
QUALIFICATIONS
- Masters of Social Work required from a school accredited by the Council on Social Work Education, unless grandfathered in per the 2008 Medicare Hospice Condition of Participation
- At least one year social work experience in a health care setting.
