SUMMARY :

Provides clinical social work services to all age patients / families in the Institute for Family Health. The scope of practice includes but is not limited to diagnostic Assessment, counseling, psychotherapy, brief treatment, behavior modification, case Management and mental health consultation.

The social worker will report to the Regional Director and work collaboratively with members of the health care team throughout the Organization

REQUIREMENTS :

• Performs clinical / technical / service / administrative tasks and participates in departmental and regional meetings as well as in the preparation and updating of policy and procedure manuals.
• Provides direct social work services, which include crisis intervention, individual, family and group counseling in collaboration with other members of an interdisciplinary team and completion of relevant documents, psychosocial evaluations or service planed case management services.
• Works in collaboration with physicians and other health care personnel in patient evaluation and treatment, to further their understanding of significant social and emotional factors underlying patient’s health problems
• Utilizes Phq9 in the assessment of patients, following agency guidelines for patients care with depression.
• Assures social services compliance with legal, regulatory and professional standards, including Article 28 and JCAHO (where applicable) and OMH
• Provides referrals for long-term counseling and for other necessary services (e.g. substance abuse treatment, legal services, housing assistance).
• Coordinates child abuse reporting with Vice President for Psychosocial Services and Regional Director

Qualifications

QUALIFICATIONS :

• Master in Social Work from accredited University
• Experience in working with a diverse populations i.e., HIV, homeless, mentally ill, elderly, children adolescents, etc
• Excellent working knowledge of behavioral medicine and evidence based treatments for medical and mental health conditions.
• Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions
• Should be comfortable with the pace of primary care, working with an interdisciplinary team and have strong communication skills
• Good knowledge of psychopharmacology
• Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions; Technical knowledge of medical social work and relevant community resources, ability to develop linkage with community organizations, as well as excellent communication and human relation skill
• Healthcare and mental health experience preferred
• Bi-lingual, Spanish preferred
• NYS License or license pending

JOB SUMMARY :

Develops and maintains an environment which supports service excellence behaviors.

Provides a safe environment for patients, visitors and / or employees by being aware of the safety and health policies of the hospital and adhering to and enhancing these policies via the identification / reporting / correcting of unsafe conditions.

Complies with established department / hospital policies, rules and regulations.

The Social Worker will assess the patient’s psychosocial problems and needs within 72 hours of admission and ongoing thereafter.

In collaboration with the patient, family / significant other and other members of the treatment team, the Social Worker will develop an initial and ongoing plan for treatment and aftercare that is based upon the identified, prioritized psychosocial problems.

The Social Worker carries out the plan for treatment and aftercare as designed, evaluates the patient’s responses to the planned interventions, and makes decisions regarding continuation or revision of the treatment plan in coordination with the treatment team

The Social Worker maintains the ability to interact with clients, families and co-workers for the purpose of fostering a therapeutic relationship with patients.

The Social Worker documents all phases of the treatment process in the medical record within appropriate time frames.

The Social Worker is clinically and administratively responsible to the Director of Mental Health Services

The Social Worker maintains accountability for clinical competency, professional growth and development and the professional ethics of clinical practice.

The Social Worker demonstrates the competencies necessary to provide services to the psychiatric patient.

The Social Worker is responsible for maintaining the appropriate documentation for the position.

MINIMUM QUALIFICATIONS :

Education :

Hold’s a Master’s Degree in Social Work from an accredited school.

Licensure / Registration / Certification :

Hold’s a license as a Certified Social Worker from the state of New York.

Work Experience :

Related experience in health / human services field strongly preferred.

Responsibilities

The primary purpose of the Social Worker I position is to be skilled in interventions for the support and / or resolution of patient and family crises as well as advocacy and facilitation necessitated by life-changing events.

The role includes other functions such as discharge planning. The position functions with considerable autonomy in providing professional social work treatment and services to patients from newborn to elderly and understands the life cycle changes consistent with each age and their families related to their health care and general well-being.

Qualifications

Possession of a Master’s Degree in Social Work (MSW) from a professional school accredited by the Council of Social Work Education and two (2) years of post MSW experience in a medical setting preferred.

Must be Licensed as an LSW Licensed Social Worker or LCSW Licensed Clinical Social Worker licensure by the State of New Jersey.

Discharge planning experience in an acute care setting highly desirable. Monday through Friday position; occasionally may be required to follow a varied schedule which may include evenings, weekends and holidays.

Alaris Health at Kearny, a 120 bed-skilled nursing Center in Kearny, New Jersey, seeks a social worker to plan and implement social service programs in the Center and coordinate discharge planning.

Additional responsibilities include :

• planning and implementing a comprehensive social services program that offers counseling and other support services for residents and their families
• evaluating and being keenly aware of residents’ medical-related social needs and then formulating the appropriate plan for treatment that fits into their total care plan
• coordinating and implementing family meetings for residents and creating a forum for ongoing discussions between residents, their families / responsible parties and the treatment team
• maintaining residents’ rights at all times while liaising between all parties involved
• coordinating discharge planning and assisting in the creation of an organized discharge plan for all residents
• and above all, ensuring that residents and their families receive the highest quality of service in a caring and compassionate atmosphere that recognizes the individuals’ needs and rights

JOB REQUIREMENTS :

• Bachelor’s degree or higher in social work or human services (knowledge of family assessment and interventions is preferred)
• Certifications required by New Jersey state regulations
• Excellent command of the English language (written, read and spoken), excellent interpersonal skills and knowledge of all governmental regulations relating to the social services department
• Ability to work independently, problem solve and makes decisions as necessary
• Ability to positively interact with everyone from residents to government agencies and the general public

Qualifications : MSW Required Bilingual English / Spanish preferred. Experience and demonstrated skill in working with youth and families required.

Prior experience in a school setting preferred. Ability to implement best practices to achieve desirable outcomes. Must be able to work occasional evenings and weekends.

Excellent organizational, written, and communication skills. Must have a commitment to work from a strength-based and / or youth development perspective.
