Position Summary
The Registered Nurse provides direct patient care. The Registered Nurse collaborates with patients, family / significant others, peers, physicians and other members of the interdisciplinary healthcare team in the delivery of healthcare services to patients.
The Registered Nurse assesses the patient’s needs, plans care, implements and evaluates the medical and nursing regimens in accordance with the hospital and nursing philosophy, policies / procedures and standards.
He or she demonstrates leadership skills and commitment to professional accountability and growth
Essential Functions and Responsibilities Includes the Following :
- Understands and adheres to the WPH Performance Standards, Policies and Behaviors.
- Utilizes the nursing process to meet the age specific needs of the patient in all aspects of care : biophysical, psychosocial, education, safety / environment and discharge planning
- Utilizes appropriate steps in the decision making process to include recognition and priority setting related to patient care and unit-based issues.
Addresses issues with appropriate actions
- Collaborates and communicates with the interdisciplinary health care team

- Adheres to infection control standards to reduce the risk of nosocomial infections to patient and staff.

Education & Experience Requirements
- Graduate from an accredited school of nursing with current New York registration
- Baccalaureate in Nursing (BSN) required for all external RNs hired on or after January 1 st , 2018.
- All RNs hired on or after January 1 st 2016 but before January 1 st 2018, must have a minimum of a BSN or show evidence of enrollment and completion of BSN by January 1 st , 2020.
- All RNs hired before January 1 st 2016 must have BSN by January 1 st , 2022.
- Knowledge of current nursing, medical and health trends and practices in rendering of patient care.
- One year of acute care nursing experience preferred for areas other than medical surgical or step-down
- BLS certification (through American Heart Association) is required.
Current nurses will have 12 months to receive this certification (1 / 1 / 18). Current nurses on Mother / Baby will have until 1 / 1 / 19 to receive this certification.
All newly hired, inexperienced nurses must receive this certification within 12 months. All newly hired, experienced nurses must receive this certification within 6 months.
PALS certification (through American Heart Association) is required for all RNs working in the PACU, 5I Pediatrics, Emergency Department, Armonk Urgent Care and Ambulatory Surgery.

- Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.

- Instructs and supervises the patient's family caregiver in the care of the patient and maintenance of a healthy environment for the patient.
- Actively participates in weekly case conferences.
- Maintains a current and accurate patient medication profile.
- After start of care (SOC) assessment, reports the status of the patient, assessed needs, and plan of care overview to the team leader on same day (or by next business day if after hours).
- Observes, records and reports to the physician and / or team leader the patient's signs and symptoms, response to treatment and changes in the patient's condition, as appropriate.
Ensures adequate Team Leader (TL) communication when physician follow-up is needed.
- Communicates changes in visit assignments, dates of scheduled visits, and schedule changes to scheduler and Patient Care Manger to ensure patient needs are met.
- Communicates timely and effectively with agency personnel and ordering physician as required to process orders and OASIS data sets, schedule home visits, and deliver services to patient as ordered by physician and in accordance with applicable laws and regulation.
- Facilitates hand-off communication to RN and PCM who will cover patients in their absence, prior to scheduled days off.
- Performs regular and supervisory visits according to the plan of care and submits complete visit notes within 24 hours of completion visit.
- Directly and / or indirectly supervises care provided by the home health aides and licensed practical vocational nurses, provides instruction as appropriate, and assigns tasks according to State and federal regulations.
- Participates in on-call rotation.
- Adheres to and participates in the agency's Episode Management process.
- Assists in the orientation of new agency personnel as assigned.
- Completes LHC required learning courses, additional assignments per DON request, as well as any state specific required
training per state regulation practice act requirements.
Participates in the performance improvement plan and process to ensure positive patient outcomes.
