About the role : Performs evidence based, outpatient orthopedic physical therapy in a collaborative, multidisciplinary Health Center with the objective of helping patients get, stay and be well.
Must be able to work independently, be comfortable with public speaking and promoting member engagement. Orthopedic Physical Therapy experience is required.
Schedule for the role :
- Tuesday and Thursday 8am-4 : 30pm with a 30 minute lunch break.
- 16 hours per week.
Essential Functions :
- Premise Health Physical Therapists work in collaboration with other Health Center staff members including, doctors, NPs, PAs, nurses, dietitians, wellness coaches, as well as community providers to promote health and wellness to onsite employees and their dependents.
- Our physical therapist practice according to APTA professional standards, recognizing the importance of current physical therapy research, critical clinical reasoning and the patient’s values when evaluating, assessing and developing a treatment plan for each unique patient.
- We successfully promote direct access to physical therapy services at the majority of our Health Centers, as such our PTs are able to practice at their highest skill level.
- Continuity of care and patient centeredness are hallmarks of the Premise Health mission and as such, our PTs are expected to demonstrate excellent interpersonal communication by participating in Health Center meetings, patient care meetings and through their documentation and reporting to onsite and community referral sources.
- Physical therapists participate in continuing competence activities including peer review, presentations, and optionally grand rounds.
- Onsite PTs may work closely with our client’s stakeholders including, safety officers, ergonomists, human resources and other vendors to promote employee safety in the workplace.
- Depending on the workplace setting and the scope of services, a Premise Health PT is able to effect positive behavior change in the eligible population, beyond the clinical setting by :
- Developing and administering injury prevention and health promotion presentations, workshops and other programing for the onsite employees and other eligible populations.
- Teaching real time proper body mechanics at the employee’s workstation. Assessing and correcting poor ergonomics.
- Developing and administering specific programs when a need is identified. Examples of programs our therapists have developed are : Weekend Warrior musculoskeletal screens, Triathlon preparation workshops, Health Back Programs, and Stretch and Flex exercise classes.
- May require other duties as assigned.
Job Requirements :
- Education : All state licenses / certifications to practice Current board certificate (if applicable)
- Degree from accredited Physical / Occupational Therapy School CPR (BLS for the Healthcare Provider / Professional Rescuer -the copy must include the front and back of card as applicable)
- Certification must include hands on evaluation of skills by the trainer / instructor.
- Total on-line courses will not be accepted.
- Course must also meet the American Heart Association (AHA) standards but need not be an AHA certificate.
Preferred Experience :
At least three years’ experience as a Physical Therapist in an outpatient physical therapy clinic or other ambulatory care clinic.
Outpatient orthopedic experience is required.

Responsibilities
Responsibilities of the Physical Therapist (PT) :
- Performs comprehensive evaluations, develops and appropriately progresses individualized treatment plans.
- Provides thorough education to patients and caregivers in a manner they can understand, and plans appropriately for discharge when medical necessity no longer indicates skilled therapeutic care.
- Participates in departmental initiatives and demonstrates effective communication with all rehab team members.

Qualifications

Overview
Provides physical therapy to VNS Home Care patients through comprehensive assessment. Develops and implements a plan of care consistent with the Agency's goals and Rehabilitation department policies and procedures.
Works under general supervision.
Responsibilities
- Conducts comprehensive assessment of patients by determining patient needs, develops courses of action including treatment plan and outcomes and implements the home physical therapy care plan.
- Maintains patient records by completing all required VNS Health documentation and ensuring compliance to Agency standards and policies.
- Monitors patient progress through assessment of care provided, including need for revision to treatment plan, outcomes and discharge plan as appropriate.
- Counsels caregivers and family by providing education and consultation for related patent care issues, including referrals within and outside of VNS Health as necessary
- Assists patients with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions, with and without assistance and as appropriate, instructing, demonstrating and verifying capabilities of family members and / or paraprofessionals.
Ability to use and assist patient with use durable medical equipment as a component of the plan of care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise / strengthening equipment.
Travels to patients’ homes and / or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single / multiple family homes, presence of pets, etc.
using approved transportation options to deliver direct care to the patient.
- Transports and utilizes required technology devices (tablet), infection control materials and supplies using VNS Health designated / supplied carrying case (backpack) weighing approximately 15 20 lbs to and from patient homes / care facilities, VNS Health offices and therapists’ home.
- Provides care in accordance with Agency policies, practices, procedures and standards, including professional practice standards of care, which may require standing, stooping, sitting, crouching, twisting, reaching, bending and stretching to deliver patient care.
- Evaluates the need for equipment by analyzing client requirements, identifying devices needed, ordering equipment and instructing patient / caregivers / family in its safe use.
- Manages assigned caseload; provides physical therapy services to patients as per plan of care.
- Assess cases for appropriateness of Physical Therapy Assistant (PTA) services. Makes recommendation if appropriate.
- Participates in VNS Health meetings including but not limited to, interdisciplinary team meetings, inservices and training programs, rehab services meetings and caseload reviews.
- Serves as Coordinator of Care (COC) on selected cases, which meet criteria for PT only cases.
- Participates in special projects and performs other duties as assigned.
Qualifications
Licenses and Certifications :
New York State License and current registration in Physical Therapy required
Valid driver's license may be required, as determined by operational / regional needs.
Education :
Bachelor's Degree in Physical Therapy from a program approved by the New York State Department of Education required
Work Experience :
Minimum of one year physical therapy experience or successful completion of the probationary period as a VNSNY Rehab Intern preferred
Basic PC skills preferred
Bilingual skills may be required, as determined by operational needs.
Pay Range per Visit :
If you are applying to the per diem per visit version of this job, the hiring range is as follows : $70- $75 per visit.
